Value of PET/CT imaging in evaluation of curative effects on lung cancer

摘要 目的研究18F-FDG PET/CT显像对评价肺癌治疗效果的价值。方法 18F-FDG PET/CT显像检查98例手术+放化疗、放化疗或氩氦刀治疗后1 w^2个月的肺癌患者,根据同机CT肺癌病灶分布特征,分析18F-FDG PET显像肺癌病灶的代谢情况。结果 98例中,PET显像提示阳性病灶87例,阴性病灶11例,其中手术治疗+放化疗组阳性34例(77.3%),放化疗组阳性14例(93.3%),氩氦刀组阳性39例(100.0%),各组18F-FDG显像病灶阳性率比较有显著差异(P<0.01)。18F-FDG PET显示病灶130个,病灶最大横径2.2~9.4(5.20±1.73)cm;同机CT显示病灶132个,最大横径1.0~10.6(4.48±2.19)cm,18F-FDG PET与同机CT显示的病灶大小基本一致(P>0.05)。18F-FDG PET病灶的T/NT比值1.3~26.07(6.32±5.48)。各组之间,PET/CT显示的治疗后残存高代谢病灶的数量有显著差异(P<0.01),其中,手术+放化疗组阳性率最低,氩氦刀组阳性率较高。结论 18F-FDG PET/CT显像对观察肺癌疗效有较大的临床价值,不仅能够准确反映治疗后病灶大小的变化情况,还能够提供肿瘤细胞的代谢信息,为临床治疗方案的调整提供可靠依据。 Objective To study the value of 18F-FDG PET/CT imaging in the evaluation of curative effects on lung cancer.Methods 98 patients with lung cancer received18F-FDG PET/CT examination plus radiotherapy and chemotherapy, or simply radiotherapy and chemotherapy, or argon helium knife treatment for one week to two months. According to the distribution characteristics of lung cancer lesions showed by spiral CT, analysis was made in the metabolism of lung cancer lesions on 18F-FDG PET imaging. Results Among the 98 cases, PET imaging indicated 87 cases of positive lesions and 11 ones of negative lesions, in which there were 34 positive cases in surgery plus radiotherapy and chemotherapy group(77.3%), 14 positive cases in radiotherapy and chemotherapy group(93.3%), and 39 positive cases in argon helium knife group(100.0%). There were significant differences in the positive rate of lesions focuses on 18F-FDG imaging among the three groups(P〈 0.01).18^F-FDG PET showed 130 lesion focuses with the largest transverse dia meter of 2.2-9.4(5.20 ±1.73)cm. CT showed 132 focuses with the largest transverse diameter of 1.0-10.6(4.48±2.19)cm. There was no significant difference between the sizes of focuses shown by 18F- FDG PET and CT(P〈 0.05).The T/NT ratio of focuses on 18F-FDG PET was 1.3-26.07(the average of 6.32 ±5.48). The differences in the numbers of remained hypermetablic focuses after the treatment indicated on PET/CT were signigicant among the groups(P〈 0.01). The positive rate of operation plus radiotherapy and chemotherapy group was the lowest, while that of the argon helium knife group was much higher.Conclusion 18F- FDG PET/CT imaging has important clinical value in the observation of curative effects on lung cancer. It can not only accurately reflect the changes in the sizes of the lesion focuses after the treatment but also provide metabolic information of tumor cells, which provides reliable basis for clinical treatment adjustment.
