腹腔感染患者免疫微生态综合肠内营养外科治疗影响的研究 被引量:1

Curative effect ofimmune microbial ecological enteral nutrition in treating patients with abdominal cavity infection

摘要 目的研究对腹腔感染患者实施免疫微生态综合肠内营养外科治疗后感染并发症与胰岛素抵抗影响。方法选择2011年5月-2015年2月140例腹部感染患者,将其随机分为研究组和对照组,各70例,对照组患者使用通用的肠内营养方法,研究组使用免疫微生态综合肠内营养方法,两组患者均给予甲硝唑和头孢曲松进行相互配合治疗,数据采用SPSS16.0软件进行统计分析。结果研究组患者腹腔内外感染率分别为12.9%、27.2%,吻合口并发症发生率为36.3%,对照组腹腔内外感染率分别为41.6%、66.5%,吻合口并发症发生率为66.5%,两组比较差异有统计学意义(P<0.05);治疗后第7天与对照组相比较,研究组患者空腹胰岛素和胰岛素抵抗指数(HOMA-IR)显著下降,差异有统计学意义(P<0.05);治疗后第7天研究组患者的CD4+/CD8+、IgG、IgM均高于对照组,与对照组和术前相比差异均有统计学意义(P<0.05)。结论免疫微生态综合肠内营养可有效地降低腹腔感染患者外科治疗后并发症的发生率,降低患者胰岛素抵抗;同时,实施免疫微生态综合肠内营养可以提高腹腔感染患者的免疫能力。 OBJECTIVE To study the influence of comprehensive enteral nutrition with immune micro‐ecology on abdominal cavity infection and insulin resistance .METHODS Totally 140 patients with abdominal infection were chosen from May 2011 to Feb .2015 during the period ,and they were randomly divided into research group and the control group with 70 cases each .The control group was given general methods of enteral nutrition ,while the research group used the immune micro ecological method combined with enteral nutrition .All patients received metronidazole and ceftriaxone sodium to coordinate the treatments .Statistical analysis of the data was conducted by SPSS16 .0 .RESULTS The research group was found to have 9 cases of intra‐abdominal cavity infection ,and the infection rate was 12 .9% ,and 19 cases of outside abdominal cavity infection with the infection rate of 27 .2% . Anastomotic complications incidence rate was 36 .3% .However ,in the control group ,the infection rates were 41 .6% and 66 .5% respectively .And the anastomotic complications incidence rate was 66 .5% .The differences were significant (P〈0 .05) .After 7 days of treatments ,the fasting insulin level and HOMA‐IR in the research group were significantly lower than those of the control group (P〈 0 .05) .And levels of C4 + /CD8 + ,IgG and IgM were all significantly higher than those of the control group (P〈0 .05) .CONCLUSION The immune micro ec‐ological comprehensive enteral nutrition can effectively reduce infection complications after surgical treatment for patients with abdominal cavity infection ,and reduce insulin resistance;meanwhile improve immunity of patients with abdominal cavity infection .
