肾结石患者经皮肾镜取石术后重症感染病原菌与对肾功能影响机制研究 被引量:21

Pathogens causing severe postoperative infections in kidney stone patients undergoing percutaneous nephrolithotomy and influence on renal function

摘要 目的调查肾结石患者经皮肾镜取石术后重症感染的病原菌分布,探讨重症感染对肾功能的影响,为临床治疗提供参考依据。方法选择2011年4月-2014年4月医院诊治的120例经皮肾镜取石术后重症感染患者进行研究,采用全自动微生物鉴定仪对病原菌鉴定,检测患者手术前后血清中丙二醛(MDA)、超氧化物歧化酶(SOD)、肌酐、尿素氮及微量蛋白尿水平,ELISA法检测血清中NADPH氧化酶亚基2(NOX2)、NADPH氧化酶亚基4(NOX4)及双功能氧化酶1(DUOX1)的水平。结果 120例经皮肾镜取石术后重症感染患者共分离出病原菌145株,其中革兰阳性菌占48.2%,革兰阴性菌占51.8%;术后重症感染患者血清中肌酐、尿素氮、微量蛋白尿及MDA水平升高(P<0.05);NOX2、NOX4及DUOX1表达增强(P<0.05),SOD水平降低(P<0.05)。结论肾结石患者经皮肾镜取石术后易发生重症感染,并导致患者肾脏功能异常和氧化应激蛋白表达增强。 OBJECTIVE To investigate the distribution of pathogens causing severe postoperative infections in the kidney stone patients undergoing percutaneous nephrolithotomy and explore the impact of severe infections on renal function so as to provide guidance for clinical treatment .METHODS A total of 120 patients with severe postopera‐tive infections who underwent the percutaneous nephrolithotomy from Apr 2011 to Apr 2014 were enrolled in the study .The pathogens were identified by using automatic microorganism identification system ,the levels of serum malondialdehyde (MDA) ,superoxide dismutase (SOD) ,creatinine ,urea nitrogen ,and microalbuminuria of the patients were determined before and after the surgery was carried out ;the levels of serum NADPH oxidase subunit 2 (NOX2) ,NADPH oxidase subunit 4 (NOX4) ,and dual‐function oxidase 1(DUOX1) were detected with the use of ELISA .RESULTS Totally 145 strains of pathogens were isolated from the 120 patients with severe postoperative infections undergoing the percutaneous nephrolithotomy , of which 48 .2% were gram‐positive bacteria , and 51 .8% were gram‐negative bacteria .The levels of serum creatinine ,urea nitrogen ,microalbuminuria ,and MDA of the patients with severe postoperative infections were increased(P〈0 .05) .The contents of NOX2 ,NOX4 ,and DUOX1 were increased (P〈0 .05) ,while the level of SOD was reduced (P〈0 .05) .CONCLUSION The patients with kidney stones are prone to have the severe infections after the percutaneous nephrolithotomy ,which may re‐sult in the abnormal renal function and the elevated level of oxidative stress protein .
