转子下截骨结合原位固定治疗青少年股骨头骨骺滑脱临床分析

Clinical Efficacy of Rotor Osteotomy Combined with in Situ Fixation for Slipped Capital Femoral Epiphysis

摘要 目的:探讨转子下截骨结合原位固定治疗股骨头骨骺滑脱的早中期临床疗效。方法:选取青少年股骨头骨骺滑脱患者5例,所有患者均行转子下截骨结合原位固定治疗。术后进行3个月、6个月、1年及1年以上随访,并行X线检查内固定情况,根据Harris评分对髋关节活动度进行综合评估。结果:所有患者均获得随访,随访时间1~1.6年,平均1.3年。所有患者髋膝关节疼痛症状逐渐消失,骨骺滑脱均未加重,髋关节Hariss由术前的(60.22±23.56)分提高至末次随访的(95.23±2.54)分。结论:转子下截骨结合原位固定能有效防止青少年股骨头骨骺滑脱进一步发展,缓解症状,改善生活质量,是治疗股骨头骨骺滑脱的有效手段,可避免远期股骨头坏死的发生。 Objective: To investigate the early and middle clinical efficacy of rotor osteotomy combined with in situ fixation for slipped capital femoral epiphysis. Methods: 5 patients with femoral epiphysis spondylolisthesis were treated by rotor osteotomy combined with in situ fixation. Parallel after 3 months, 6 months, 1 year and over 1 year follow-up, parallel X-ray view of internal fixation, and to conduct a comprehensive assessment of the activity according to Harris hip score. Results: All patients were fol- lowed up for 1 -- 1.6 years, an average 1.3 years. All patients hip and knee pain gradually disappeared, epiphyseal slippage are in- creased, hip Hariss increased from (60.22 ± 23.56) points preoperatively to last follow-up (95.23±2.54) points. Conclusion: Ro- tor osteotomy combined with in situ fixation can effectively prevent the further development of youth slipped capital femoral epiphy- sis, and relieve the symptoms improve quality of life, it is an effective means of treatment of slipped capital femoral epiphysis, to a- void necrosis of the femoral head forward.
