摘要
目的：探讨MSCT对结直肠癌诊断的应用价值.材料与方法：回顾性分析了我院2012年3月～2014年9月经手术治疗、病理证实的结直肠癌32例患者的病例资料.术前对所有患者行MSCT平扫和三期扫描,并做图像后处理,将MSCT表现与手术和病理结果进行比较.结果：本组病理为黏液腺癌6例,中管状腺癌22例,绒毛状腺癌2例、混合型腺癌2例.CT表现,升结肠癌8例,横结肠癌2例,降结肠癌6例,乙状结肠癌3例,直肠癌13例.肿瘤形成肠腔狭窄和形态不规则21例,形成腔内肿块11例;局部肠壁增厚32例;CT上表现15例显示为浆膜面受侵,手术发现17例浆膜面受侵,淋巴结转移者CT发现18例,病理证实22例.结论：MSCT的多期扫描结合多种后处理技术,不但能明确结直肠癌病灶的部位、大小,还能明确病变浸润深度、病变与周围脏器的关系以及有无邻近淋巴结转移或（和）其他脏器转移等,为临床大夫术前正确的评价病变并决定手术方式提供了准确客观依据.
Purpose： Discuss value of MSCT in the diagnosis of colorectal Cancer. Materials and Methods： The clinical data of 32 cases of colorectal cancer confirmed by surgery and pathology were analyzed retrospectively from March 2012 to September 2014. MSCT scan and threephase scan were performed in all patients before surgery, and after dong image pro- cessing, compared with operative and pathological findings . Results： The pathologic types of the case as follows ： mucinous adenocarcinoma in 6 patients, the tubular adenocarcinoma in 22, villoglandularadenocarcinoma in 2 and mixed adenocarcino- ma in 2. CT is shown ascending Colorectal Cancer in 8patients, transverse colorectal cancer in 2, descending colorectal cancer in 6, sigmoid colon cancer in3 and rectal cancer in 13. Tumour formats luminal stenosis and irregular in 21 and formats in- traluminal mass in 11 ; partial wall thickening in 32; CT image shows serosa invasion in 15patients. Surgery found serosa inva- sion in 17 patients, and lymph node metastasis in 18 from CT images, pathologically confirmed 22 cases. Conclusion： Multi- phase scan of MSCT combined with a variety of image processing technology not only can clear the part and the size of the le- sion of colorectal cancer , but also can clear lesions invasion, relationship between lesion and surrounding organs and metasta- sis of lymph nodes or （and） metastasis of other organs , etc. , which provides accurate objective basis for clinicians to preop- eratively assess lesions and decide treatment option.
