HPV16 E6基因和蛋白水平与维汉宫颈病变进展的关系研究 被引量:6

摘要 目的:评价 HPV16 DNA E6载量和蛋白表达与宫颈病变病情进展的关系,并比较维、汉之间的差异性。方法:通过收集妇科门诊就诊的 HPV16感染的维吾尔族和汉族各个级别宫颈病变妇女的宫颈组织和脱离细胞。采用实时荧光定量 PCR 测定HPV16 DNA E6载量,免疫组化法检测 HPV16 E6蛋白表达。结果:汉族和维吾尔族中,炎症组、CIN1、CIN2~3、鳞癌组4组的 HPV16 E6 DNA 载量比较,差异均有统计学意义(H=30.33,P =0.001;H=37.37,P =0.001)。 CIN2~3 组中,维吾尔族的 HPV16 E6 DNA 载量高于汉族,差异有统计学意义(Z =-2.266,P =0.023)。汉族和维吾尔族中,炎症组、CIN1、CIN2~3、鳞癌组4组的 HPV16 E6蛋白表达比较,差异有统计学意义(χ2=38.85,P=0.001;χ2=44.71,P =0.001);HPV16 E6蛋白表达随着病变程度的增加呈上升趋势。炎症、CIN1、CIN2~3 中,HPV16 E6 DNA 载量和蛋白表达具有明显的正相关性( P 〈0.05);宫颈癌中 HPV16 E6 DNA 载量和蛋白表达无相关性(P〉0.05)。结论:维吾尔族CIN2~3 患者的 HPV16 E6 DNA 载量高于汉族 CIN2~3 患者,E6基因在维吾尔族宫颈癌发生可能有更重要作用,HPV16 E6 DNA 载量可成为预测和诊断宫颈癌前病变和宫颈癌的标志物。 Objective:To evaluate the relationship between HPV16 E6 DNA load and protein expression levels and cervical lesion progression,and to compare the differences between Uygur and Han. Methods:Cervical tissues and shedding cells which were HPV16 positive in-cluding 4 pathological levels from Uighur and Han,respectively were collected from the gyneco-logical clinic. Using multiple real-time PCR to detect HPV DNA E6 load,and Using Immune histochemical method to inspect the expression of HPV16 E6 protein. Results:HPV16 E6 DNA load were statistically significant differences among inflammation group,CIN1,CIN2 ~ 3 and carcinoma group in Uighur and Han,respectively (H = 30. 33,P = 0. 001,and H = 37. 37,P =0. 001);In the CIN2 ~ 3 group,the HPV16 E6 DNA load of Uighur was higher than that of the Han,and the difference was statistically significant (P = 0. 023,Z = -2. 266);HPV16 E6 pro-tein expression were statistically significant differences among inflammation group,CIN1,CIN2~ 3 and carcinoma group in Uighur and Han,respectively (χ2 = 38. 85,P = 0. 001,and χ2 =44. 71,P =0. 001);as the disease progressed,HPV16 E6 protein expression was on the rise. Between the expression of E6 protein and E6 DNA load of HPV16 was positively correlated in inflammation,CIN 1 and 2 ~ 3 CIN (P〈0. 05),and there was no correlation in cervical cancer(P〉0. 05). Conclusion:in Uygur patients with CIN2 ~ 3 HPV16 E6 DNA load is higher than that of Han,E6 gene in Uigur cervical cancer may have a more important role,and HPV16 E6 DNA load can be used as a marker for prediction and diagnosis of cervical precancerous lesions and cervical cancer.
