摘要
目的 探讨血细胞形态学检测在发热疾病诊断中的应用价值,从而为该疾病的有效诊断提供科学的理论依据。方法 以我院在2013年12月~2014年12月收治的50例发热患者作为实验组,选择正常体检检查的50例无发热患者作为对照组,对所有患者血液样本进行血涂片染色显微镜检查和血细胞常规分析,对比观察两组受检者的血细胞形态学检查指标的阳性率,其指标包括异常血细胞形态的阳性比例以及红细胞分布宽度标准差（RDW-SD）、平均细胞体积（MCV）、平均红细胞血红蛋白浓度（MCHC）、血细胞比容（Hct）、平均红细胞血红蛋白（MCH）等。结果 实验组的异常细胞、白细胞变化、淋巴细胞变化、细胞性贫血分别占16.0%、18.0%、12.0%、12.0%,均明显高于对照组（P〈0.05）;实验组的RDW-SD、MCV、MCHC、Hct、MCH分别为（13.4±1.4）%、（87.3±10.4）fl、（361.4±13.6）g/L、（44.5±3.4）%、（33.5±4.4）pg,均明显高于对照组（P〈0.05）。结论 血细胞形态学检验能够有效反映发热患者的病情,并为该疾病的诊断提供准确的实验室依据,值得临床进一步推广应用。
Objective To investigate the application value of detection of blood cell morphology in diagnosis of with febrile disease to provide scientific theoretical basis for the effective diagnosis of this disease. Methods Fifty patients with fever admitted into our hospital from December 2013 to December 2014 were selected as experiment group and 50 patients without fever who received normal physical examination were selected as control group.The blood samples of all patients were given blood smear staining microscopic examination and blood cell routine analysis.The positive rate of blood cell morphological assay indicators of between two groups was observed comparatively respectively, and the indicators included positive ratio of abnormal blood cell morphology and standard deviation of red blood cell distribution width （RDW-SD）,mean cell volume （MCV）,mean corpuscular hemoglobin concentration （MCHC）,hematoerit （Hct） and mean corpuscular hemoglobin （MCH）. Results In experiment group,abnormal cell,leukocyte change,lymphocyte changes and cell anemia accounted for 16.0%, 18.0%, 12.0% and 12.0% respectively,which was significantly higher than that of control group respectively （P〈0.05）.In experiment group,RDW-SD,MCV,MCHC,Hct and MCH was （13.4~1.4）%,（87.3~ 10.4） fl,（361.4＋_13.6） g/L,（44.5＋3.4）% and （33.5＋4.4） pg respectively,which was significantly higher than that in control group （P〈0.05）. Conclusion Detection of blood cell morphology can effectively reflect the state of fever and provide laboratory reference for the accurate diagnosis of this disease.h is worthy of clinical promotion and application.
