摘要
目的 探讨多巴酚丁胺联合硝酸甘油治疗慢性顽固性心力衰竭的效果。方法 随机抽取2012年3月~2015年1月我院收治的122例患者的临床资料。按照随机数字表法将患者均分为观察组（61例）和对照组（61例）。对照组采用硝酸甘油治疗,观察组采用多巴酚丁胺联合硝酸甘油治疗。观察两组患者的临床疗效、症状缓解时间和住院时间、BNP水平变化情况、不良反应发生情况。结果 观察组的总有效率显著高于对照组,差异有统计学意义（P〈0.05）。观察组的症状缓解时间和住院时间均短于对照组,差异有统计学意义（P〈0.05）。治疗前,观察组的BNP水平与对照组比较,差异无统计学意义（P〉0.05）;治疗后,观察组的BNP水平与对照组比较,差异有统计学意义（P〈0.05）;治疗后,两组的BNP水平与同组治疗前比较,差异有统计学意义（P〈0.05）。两组患者均未出现严重不良反应。两组患者的不良反应发生率比较,差异无统计学意义（P〉0.05）。结论 多巴酚丁胺联合硝酸甘油治疗慢性顽固性心力衰竭的效果佳,可有效缩短患者的住院时间,快速缓解临床症状,降低患者的BNP水平,且无明显不良反应。
Objective To explore the effect of dobutamine combined with nitroglycerin treating chronic refractory heart failure. Methods Clinical data of 122 patients with chronic refractory heart failure treated in our hospital from March 2012 to January 2015 was randomly extracted.Patients were randomly divided into observation group （61 cases） and control group （61 cases） according to the random number table method.Control group was treated with nitroglycerin, while observation group was treated with dobutamine combined with nitroglycerin.The clinical effect,the time of symp- tom relieving,hospital stay,change of BNP level,incidence of adverse reaction of patients of two groups was compared respectively. Results The total effective rate of observation group was higher than that of control group,and there was a statistical difference （P〈0.OS）;the time of symptom relieving,hospital stay in observation group was shorter than that of control group respectively,and there was a statistical difference （P〈0.05）.There was no statistical difference of BNP level in treatment group before treatment compared with control group （P〉0.05）;there was a statistical difference of BNP level in treatment group after treatment compared with control group （P〈0.05）;there was a statistical difference of BNP level in the two groups after treatment compared with before treatment respectively （P〈0.05）. There has no serious adverse reaction in two groups.There has no statistical difference of incidence rate of adverse reaction in the two groups （P〉 0.05）. Conclusion Dobutamine combined with nitroglycerin treating chronic refractory heart failure has good effect and can effectively shorten hospital stay and relieve rapidly clinical symptom,and decrease BNP level of patients,and it has no obvious adverse reaction.
