摘要
目的 观察注射型唑来膦酸盐对骨质疏松患者生活质量、骨代谢指标的影响。方法 将九江市第一人民医院2013年2月~2014年2月收治的110例骨质疏松患者按随机数字表分为观察组55例和对照组55例,对照组给予钙剂与活性维生素D治疗,观察组在对照组的基础上再给予注射型唑来膦酸盐治疗,5 mg/次,1次/年,治疗前后空腹抽血,检测骨代谢指标,采用QUALEFFO-41量表评价生活质量改善情况。结果 观察组治疗后碱性磷酸酶（ALP）、N-骨钙素（N-MID）、β-胶原片段（β-CTX）、Ⅰ型前胶原氨基末端肽（PⅠNP）较治疗前降低,差异有统计学意义（P〈0.01）,对照组治疗前后骨代谢指标差异无统计学意义（P〉0.05）,观察组治疗后ALP、N-MID、β-CTX、PⅠNP明显低于对照组,差异有统计学意义（P〈0.01）。观察组治疗后QUALEFFO-41量表中的疼痛、日常活动、家务劳动、日常运动、社会功能、总体情况、精神状态评分及总积分均较治疗前明显降低（P〈0.05）,对照组治疗前后上述指标无明显变化（P〉0.05）。结论 注射型唑来膦酸盐能有效改善患者生活质量及骨代谢指标,抑制破骨细胞活性,减少骨吸收,但其价格昂贵,临床医师应在药物疗效和经济性上进行合理选择。
Objective To observe the influence of zoledronate on life quality and bone metabolism in the treatment of patients with osteoporosis. Methods 110 patients with osteoporosis from the First People＇s Hospital of Jiujiang City were randomly divided into 55 cases of observation group and 55 cases of control group.The control group were received calcium and active vitamin D,the observation group were given injectable azole group on the basis of control group,5 rag/time,1 time/year,the bone metabolism was detected and the life quality was evaluated by QUALEFFO-41. Results The ALP,N-MID,13-CTX,P I NP of observation group after treatment was statistically lower than before treat- ment respectively,but the bone metabolism of control group before and after treatment was not statistically significant（P〉 0.05）,theALP, N-MID,J3-CTX,P I NP of observation group after treatment was statistically lower than that of control group respectively,the difference was statistically significant（P〈0.01）.The score of pain,daily activities,household chores, daily exercise,social function,the overall situation,mental state score and total score of observation group after treatment was significantly lower than before treatment respectively （P〈0.05）.The control group before and after treatment showed no significant change in the above（P〉O.05）. Conclusion Zoledronate can improve the life quality and bone metabolism, inhibit osteoclast activity,reduce bone resorption,but the clinicians should make a reasonable choice for its efficacy and economy,which has expensive treatment costs.
