摘要
目的 探讨内镜与腹腔镜联合治疗老年胆囊结石合并胆总管结石的临床效果。方法 选取2012年3月~2014年12月本院收治的130例老年胆囊结石合并胆总管结石患者作为研究对象,采用随机数字表法分为研究组和对照组,各65例。研究组采用内镜下乳头括约肌切开术（EST）＋取石网篮取石术＋腹腔镜胆囊切除术（LC）治疗,对照组采用传统开腹胆囊切除术＋胆总管切开取石T管引流术。比较两组的手术时间、术中出血量、排气时间、肠鸣音恢复时间、住院时间及并发症发生率。结果 两组的手术时间比较,差异无统计学意义（P〉0.05）。研究组的术中出血量显著少于对照组,排气时间、肠鸣音恢复时间及住院时间显著短于对照组,差异有统计学意义（P〈0.05）。研究组的并发症发生率为9.2%,显著低于对照组的24.6%,差异有统计学意义（P〈0.05）。结论 内镜与腹腔镜联合治疗老年胆囊结石合并胆总管结石效果显著,具有创伤小、出血少、并发症少及恢复快等优点,可促进患者早日康复。
Objective To explore the clinical effect of endoscope combined with laparoscope in the treatment of elderly patients with gallstone and choledocholithiasis. Methods 130 elderly patients with gallstone and choledocholithiasis from March 2012 to December 2014 in our hospital were selected and randomly divided into the research group and the control group according to the random number table method,65 cases in each group.The research group was treated with endoscopic sphincteropapillotomy （EST）,basket extraction of stone removal and laparoscopic cholecystectomy （LC）,tbe control group was treated with the traditional open cholecystectomy combined with choledocholithotomy-T-tube drainage.The operation time,intraoperative amount of bleeding,exhaust time,time of borborygmus recovery,hospital stay and the incidence rate of complication in two groups was compared. Results There was no significant difference of the operation time between two groups （P〉0.05）.The intraoperative amount of bleeding in the research group was less than that in the control group,the exhaust time,time of borborygmus recovery,hospital stay in the research group was shorter than that in the control group,with significant difference （P〈0.05）.Tbe incidence rate of complication in the research group was lower than that in the control group,with significant difference （P〈0.05）. Conclusion Endoscope combined with laparoscope in the treatment of elderly patients with gallstone and choledocholithiasis is significant,which has the advantages of less trauma,less bleeding,less complications and quicker recovery,can enhance patient＇s early recovery.
