不同定位方法在口腔颌面深部间隙异物取出术中的效果比较 被引量:1

摘要 目的探讨不同定位方法在口腔颌面深部间隙异物取出术中的应用价值。方法选取2008年1月~2014年4月我院收治的40例口腔颌面深部间隙存在异物的患者作为研究对象,按随机分配的方式,将其分为对照组和观察组,每组20例。对照组患者采取常规的影像学定位方法进行异物取出术,观察组患者在光学导航下对患者进行异物取出术。术前对患者进行CT检查和血管造影检查,并对深部间隙的异物进行定位。在手术过程中对导航系统的准确性和实时性进行观察,并对影像学定位和光学导航在口腔颌面深部间隙异物取出中的应用效果进行评价。结果对照组患者的异物取出率为60%,观察组患者的异物取出率为90%,观察组患者的异物取出率明显高于对照组(P〈0.05)。观察组患者的异物取出时间明显短于对照组(P〈0.05)。对照组患者手术后,有4例患者出现并发症,并发症发生率为20%,观察组患者手术后均未出现并发症,观察组患者的并发症发生率明显低于对照组(P〈0.05)。结论光学导航下对口腔颌面深部间隙存在异物的患者行异物取出术,能对异物进行准确的定位,从而减少手术风险,提高手术的成功率,有效减轻患者痛苦,预防术后并发症的发生,有利于患者的预后。 Objective To evaluate the application value of different techniques in the foreign body removal surgery in oral and maxillofacial deep interval. Methods Forty patients with foreign body in oral and maxillofaeial deep interval who were treated in our hospital from January 2008 to April 2014 were selected as the study subjects and divided into control group and observation group according to the random allocation method,with 20 patients in each group.The con- ventional imaging positioning method was used for foreign body removal surgery in control group and the optical navi- gation method was used for foreign body removal surgery in observation group.Before surgery,the patients were given CT examination and angiography examination,and the foreign body in deep interval was located.During the surgical process, the accuracy and timeliness of the navigation system were observed and the application effects of imaging positioning and optical navigation in the foreign body removal surgery in oral and maxillofacial deep interval were evaluated. Re- suits The foreign body removal rate of control group was 60% and that of observation group was 90%;the foreign body removal rate of observation group was significantly higher than that of control group (P〈0.05).The foreign body removal time of theobservation group was significantly shorter than that of control group (P〈O.05).After surgery,complications oc- curred in 4 patients of the control group,with the incidence of complications of 20%,and no complication occurred in observation group;the incidence of complications of observation group was significantly lower than that of control group (P〈0.05). Conclusion The application of foreign body removal surgery under optical navigation in the patients with for- eign body in oral and maxillofaeial deep interval can locate the foreign body accurately,thus reduce surgical risk,im- prove the surgical success rate,effectively relieve patients' sufferings and effectively prevent the occurrence of postoper- ative eomplieations,whieh is conducive to the patients' prognosis.
