急性心肌梗死CD_(34)与心功能及冠状动脉狭窄相关性研究 被引量:2

Correlation Analysis of CD_(34) cells With Heart Function,Coronary Stenosis in Acute Myocardial Infarction Patients

摘要 目的 探讨急性心肌梗死患者外周血CD34与心功能及冠状动脉狭窄的相关性和临床意义。方法 选择2011年5月至2012年10月在内蒙古自治区人民医院心内科住院治疗的急性心肌梗死患者76例,根据Killip心功能分级分为KillipⅠ级组(29例)和KillipⅡ-Ⅳ级组(47例),同期选择30例正常体检者作为对照组。采用流式细胞仪检测3组受试者外周血CD34细胞数量,入院2周内急性心肌梗死患者行心脏彩色多普勒超声检查,测定左心室射血分数(LVEF),行冠状动脉造影,Gensini积分法计算冠状动脉狭窄程度。结果 KillipⅠ级组和KillipⅡ-Ⅳ级组CD34细胞数分别为(681±267)个/μL、(919±310)个/μL,均高于对照组的(214±23)个/μL,差异有统计学意义(P〈0.05),且KillipⅡ组CD34细胞数高于KillipⅠ级组(P〈0.05)。KillipⅡ-Ⅳ级组Gensini积分显著高于KillipⅠ级组[(48±21)分比(30±12)分],差异有统计学意义(P〈0.01)。CD34细胞数与LVEF呈负相关(r=-0.468,P=0.000),与Gensini积分呈正相关(r=0.301,P=0.008)。结论 检测CD34细胞数量可判断急性心肌梗死患者的心功能和冠状动脉狭窄的程度,为临床判断预后提供依据。 Objective To investigate the clinical significance and correlation between peripheral blood CD34 cell number and coronary stenosis of patients with acute myocardial infarction( AMI). Methods A total of 76 AMI patients of the Department of Cardiology of Inner Mongolia People's Hospital from May 2011 to Oct. 2012,were included in the study,and another 30 people for health examination during the same period were also included as control group. The 76 patients were divided into KillipⅠ group( 29 cases) and KillipⅡ-Ⅳ group( 47 cases) according to Killip heart function. The general information and history of diseases and electrocardiogram were collected. CD34 cell count in peripheral blood was determined by flow cytometry,cardiac color Doppler ultrasonography were completed within two weeks to determine left ventricular ejection fraction( LVEF). Coronary angiography was done. Gensini integration was used to calculate the degree of coronary stenosis. Results The number of CD34 cells of KillipⅠ group and KillipⅡ-Ⅳ group were( 681 ±267) count / μL and( 919 ± 310) count / μL,which were higher than the control group's( 214 ± 23) count / μL with statistically significant difference( P 〈0. 05),and the number of CD34 cells of Killip Ⅱ-Ⅳ group was higher than KillipⅠ group with statistically significant difference( P 〈0. 05). Gensini integral of Killip Ⅱgroup was significantly higher than KillipⅠ group [( 48 ± 21) scores vs( 30 ± 12) scores],the difference was statistically significant( P 〈0. 01). The number of CD34 cells was negatively correlated with LVEF( r =- 0. 468,P = 0. 000),was positively correlated with Gensini integral( r = 0. 301,P = 0. 008). Conclusion The number of peripheral blood CD34 cell can be used to assess the cardiac function and coronary artery stenosis degree of AMI patients,provding reference for the prognosis judgment.
