摘要
采用RIA法对27例肾移植手术一个月后,患者口服不同剂量的环孢素A(cyclosporin A.)进行全血药物浓度监测135人次,发现用药剂量与血药峰浓度及谷浓度有良好的线性关系,其中谷浓度Cmin(ng/ml)与剂量D(mg/kg/d)有D=0.0129Cmin—2.4492,r=0.948。随着血清肌酐(SCr)值增加,环孢素A血药浓度降低,表明临床已注意到环孢素A的肾毒性。根据本文结果提出肾移植手术一个月后理想的环孢素A维持浓度为500—700ng/ml(谷值,全血),其相应的剂量范围为4.00—6.58mg/kg/d。
Blood concentration monitoring of cyclosporin A was per-formed one month later after renal transplantation in 27 patients. Alto-gether 135 whold blood samples were measured by using RIA method. Themaximum and minmium drug concentration (ng/ml) have a good linearrelationship with the dosage (mg/kg/d) of cyclosporin A, eg D=0. 0129Cmin- 2 .4492 (r=0. 984). While serum Cr increasing, cyclosporin A con-centration in whole blood decreased. It means that the clinicians havepaid attention to avoid the nephrotoxicity of cyclosporin A by reducingthe dose. According to our results it was considered that the ideal maint-enance concentration range of cyclosporin A in renal transplantationpatients after one month of operation is 500-700ng/ml, the correspondingdosage range is 4 .00-6 .58mg/kg/d.
