摘要
目的:观察超声引导下胎盘局部注射甲氨蝶呤(Methotrexate,MTX)联合中药治疗胎盘植入的临床疗效。方法:将105例植入性胎盘患者随机分为两组,对照组55例和治疗组50例。两组均在超声引导下向植入的胎盘组织内局部注射MTX,治疗组在对照组的基础上联合自拟中药益气消癥汤治疗,两组均以1个月为1个疗程,共3个疗程。结果:治疗组有效率为82.00%,对照组有效率为70.91%,两组比较,差异有统计学意义(P<0.05);其在绒毛膜促性腺激素降至正常时间、胎盘排出时间、3周后宫腔胎盘残留体积及MTX注射次数均优于对照组,两组比较,差异均有统计学意义(P均<0.05)。结论:超声引导下胎盘局部注射MTX联合中药是治疗胎盘植入较好的保守治疗方法,能最大限度的保留生育能力,并且安全性高。
Objective：To observe the clinical effect of Ultrasound guided placenta local injection Methotrexate（MTX）combined with Yiqi Xiaozheng Tang Treatment of placenta implantation. Methods：105 patients with placenta increta were randomly divided into two groups,55 cases in the control group and 50 cases in the treatment group. Two groups were in ultrasound guided to the placenta in local injection of MTX,and the treatment group was treated with self-made traditional Chinese medicine Yiqi Xiaozheng Tang based on the control group. Two groups were 1 month for a course of treatment,a total of 3 courses of treatment. Results：the effective rate of treatment group was 82. 00% ,and the control group was 70. 91% ,and the difference was statistically significant（P ﹤ 0. 05）;human chorionic gonadotropin dropped to normal time,placenta discharge time,3 weeks after uterine placenta residual volume and MTX injection time in the treatment group were superior to the control group,and the difference was statistically significant（all P ﹤ 0. 05）. Conclusion：Ultra-sound guided placenta local injection MTX combined with Yiqi Xiaozheng Tang is an ideal conservative treatment for placenta implanta-tion,and it can preserve fertility maximum with high safety.
