摘要 目的应用CDFI探讨女性不同生理阶段乳腺主要供血动脉影像解剖学特征。方法选取150例健康女性,根据不同生理阶段分为育龄组、绝经组及妊娠或哺乳期组各50例,应用CDFI观察乳腺内胸外侧动脉及胸廓内动脉主要穿支的形态、走行及分布等特征,比较各组血管分级、内径、峰值流速及阻力指数情况。结果妊娠或哺乳期组可观察到Ⅳ~Ⅵ级血管分级,血管直径最宽(2.20±0.20)mm,峰值流速最高(38.62±16.38)cm/s,阻力指数最低0.52±0.06;绝经组仅观察到Ⅰ~Ⅱ级血管分级,血管直径最细(1.04±0.07)mm,峰值流速最低(13.27±3.18)cm/s,阻力指数最高0.76±0.03;育龄组所获得的参数介于上述两组之间,三组间血管分级、内径、峰值流速及阻力指数比较差异有统计学意义(P〈0.05)。结论应用CDFI可动态、直观地显示乳腺主要供血动脉形态、走行及分布等解剖学特征,充分显示女性在不同生理阶段乳腺主要供血动脉的不同变化。 Objective To explore the anatomical features of main breast-feeding arteries in different physiological stages of women by CDFI.Methods One hundred and fifty healthy women in different physiological stages were selected and divided into three groups,including child-bearing age group,postmenopausal group and pregnant or lactational group,each containing 50 members.CDFI was used to observe the morphology,course and distribution of lateral and internal breast arteries perforators,and the relationships among vascular grade,diameter,peak speed(PS) and resistance index(RI) were analyzed.Results Ⅳ~Ⅵ vascular grades in pregnant or lactational group were observed and the widest diameter was(2.20±0.20)mm,the highest PS was(38.62 ±16.38)cm/s,and the lowest RI was 0.52 ±0.06. Ⅰ ~ Ⅱ vascular grades in postmenopausal group were observed and the shortest diameter was(1.04±0.07)mm,the lowest PS was(13.27±3.18)cm/s and the highest RI was 0.76±0.03.The results in child-bearing age group were between the other two groups.There were significantly differences in vascular grade,diameter,PS and RI among the three groups(P〈0. 05). Conclusion The anatomic features of main breast- feeding arteries such as morphology features,direction and distribution could been dynamically and directly observed using CDFI.
