无锡市副溶血性弧菌感染的分子流行病学特征分析 被引量:4

Molecular epidemiological characteristics of Vibrio parahaemolyticus infection in Wuxi

摘要 目的了解无锡市腹泻人群中副溶血性弧菌感染的流行状况及分子特征。方法对无锡市30家哨点医院感染性腹泻患者的粪便标本分别进行副溶血性弧菌的分离鉴定、血清学分型、毒力基因检测以及脉冲场凝胶电泳(PFGE)图谱分析。结果在1 238例标本中共分离到31株副溶血性弧菌,阳性率为2.50%;检出时间主要集中在7-9月,以8月份的检出率最高。使用PCR方法分别检测31株副溶血性弧菌的跨膜转录激活蛋白基因(tox R)、耐热直接溶血素基因(tdh)和耐热相关溶血素基因(trh),毒力基因tox R+、tdh+、trh+携带率分别为100%、100%和3.23%。PFGE图谱显示,27株O3﹕K6血清型的副溶血性弧菌共有4种PFGE带型,88.89%的副溶血性弧菌为同一种带型。结论无锡市副溶血性弧菌的感染具有季节性和人群分布的差异性,主要由同一种毒力强的O3﹕K6型菌株引起,具有产生暴发流行的可能,需进一步加强监测管理。 Objective To study the prevalence and molecular characteristics of V.parahaemolyticus infecting patients with diarrhea in Wuxi. Methods V.parahaemolyticus was isolated from patients with infectious diarrhea at 30 sentinel hospitals with real-time PCR using the TaqMan system and it was then identified.V.parahaemolyticus was serotyped,virulence genes were detected,and pulsed-field gel electrophoresis(PFGE)was used to analyze the strains of V.parahaemolyticus. Results In total,31 strains of V.parahaemolyticus were detected in 1 238specimens(rate of detection:2.5%)from patients with infectious diarrhea.The infection occurred more frequently between July and September and adults ages 21-60 years were most affected.There were no significant differences between males and females in the rate of infection.The virulence gene tox R was found in 100.00% of the 31 strains,tdh was found in 100.00%,and trh was found in 3.23%.Based on PFGE,27 strains of the O3:K6serotype of V.parahaemolyticus were divided into 4different genotypes;88.89% of the strains had a genotype that was 100%identical. Conclusion The V.parahaemolyticus infecting people in Wuxi was mostly the highly pathogenic O3:K6serotype.V.parahaemolyticusinfection had seasonality and affected populations differently.An outbreak of V.parahaemolyticusis a possibility,so the surveillance of V.parahaemolyticus needs to be enhanced.
