摘要
目的探讨食管支架治疗食管纵隔瘘的并发症对其远期疗效的影响及治疗策略。方法对37例食管纵隔瘘患者依其瘘口位置及纵隔感染程度等具体情况实施支架置入、胃造瘘、营养管置入和肠外营养等不同治疗手段。结果 37例患者共行食管支架置入9例,胃造瘘10例,十二指肠营养管置入术15例,3例患者仅行肠外营养。结论食管纵隔瘘的治疗应依据瘘口位置及纵隔感染程度等具体情况实施不同治疗手段。食管支架必须慎重应用。
Objective To investigate the influence to the prostecdtive efficacy of treating the esophageal mediastinal fistula with intraesophageal stent and research the strategy. Methods Totally 37 patients with esophageal mediastinal fistula were treated with intraesophageal stent,gastrostomy,nutrient canal insertion and parenteral alimentation according to the fistula localization and mediastinum gradient of infection. Results In all of the 37 patients,9 patients were implanted stent,10 patients underwent gastrostomy and 15 patients got the nutrient canal insertion,3 patients were only treated by parenteral alimentation. different according to the orificium fistula localization and mediastinum. Conclusion The treatment of esophageal mediastinal fistula should be gradient of infection. Intraesophageal stent must be used judiciously.
