前列腺癌诊断中核磁弥散成像及ADC值的应用价值 被引量:3

Value of diffusion-weighted imaging and apparent diffusion coefficient in the diagnosis of prostate cancer

摘要 目的:探讨核磁共振弥散加权成像(DWI)及表观弥撒系数(ADC)在前列腺癌诊断中的应用价值。方法:回顾性分析我院自2009年1月2013年12月95例行DWI检查后经穿刺活检或外科手术证实为前列腺癌或BPH患者的临床及影像学资料,分别对不同b值下前列腺癌与BPH组织ADC值进行比较,并分析前列腺癌ADC值与肿瘤Gleason评分、ADC值与血清前列腺特异性抗原(PSA)之间的相关性。结果:95例患者中,b为600s/mm2时,前列腺癌与BPH患者中央腺体的平均ADC值分别为(1.31±0.35)×10-3 mm2/s和(1.55±0.17)×10-3 mm2/s,外周带平均ADC值为(1.20±0.38)×10-3 mm2/s和(1.42±0.23)×10-3 mm2/s;b为1 000s/mm2时,前列腺癌与BPH患者中央腺体的平均ADC值分别为(1.12±0.36)×10-3 mm2/s和(1.39±0.16)×10-3 mm2/s,外周带平均ADC值为(1.04±0.35)×10-3 mm2/s和(1.25±0.19)×10-3 mm2/s。当b为1 000s/mm2时,前列腺癌ADC值与其Gleason评分有相关性,为负相关(r=-0.346,P=0.025);还与血清PSA水平有相关性,为负相关(r=-0.315,P=0.026)。结论:ADC值是区别患者前列腺癌和BPH的重要指标,并且可以预测前列腺癌的生物学特性。 Objective: To discuss the value of diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) in the diagnosis of prostate cancer. Method: We reviewed the data of 95 patients of autonomous region people's hospital from January 2009 to December 2013. All the patients were confirmed by biopsy or surgery for prostate cancer or BPH after DWI. We compared ADC value between prostate cancer tissues and BPH samples in different b-value respectively, and analyzed the correlation between ADC value and tumor Gleason score, and the correlation between ADC value and the serum PSA. Result: When b was 600 s/mm2 , the average ADC value of central gland of prostate cancer andBPH samples were (1. 31±0. 35)X10 amm2/s and (1. 55±0. 17)X10 a mm2/s respectively, and the average ADC value of peripheral zone were (1.20±0.38) x 10 ammZ/s and (1.42± 0.23) X 10 ^3mm2/s respectively. When b was 1 000 s/mma the average ADC value in central gland were (1.12±0.36) X 10 a mm2/s and (1.39 ±0.16)X 10 ^-3mm2/s respectively, and the average ADC value for peripheral zone were (1.04±0.35) X 10^-3mmZ/s and (1.25±0. 19) X 10-3mm2/s respectively. There was a negative correlation between ADC value and Gleason score of prostate cancer (r=0. 346, P=0. 025). Also, a negative correlation between ADC value and the serum PSA level (r= -0. 315, P=0. 026) was found. Conclusion: ADC value is an important index of differential diagnosis between prostate cancer and BPH, and can predict the biological charac- teristics of prostate cancer.
