空心加压螺钉治疗成人股骨颈骨折术后股骨头坏死高危因素分析 被引量:10

摘要 目的探讨空心加压螺钉治疗成人股骨颈骨折术后股骨头坏死(ONFH)危险因素。方法选取2013年9月—2015年3月实施空心加压螺钉治疗的成人股骨颈骨折患者120例为研究对象,术后随访,观察ONFH发生情况,并分析不同因素对ONFH发生率的影响。结果治疗后,患者Harris评分显著提高,与治疗前相比差异有统计学意义(P<0.05)。多因素logistic回归分析显示,术后并发ONFH的独立危险因素包括年龄(OR=2.48,P<0.05)、有无取出内固定(OR=10.11,P<0.01)、复位质量(OR=4.23,P<0.05)、Garden分型(OR=1.64,P<0.01)。结论影响空心加压螺钉治疗股骨颈骨折术后ONFH的危险因素有年龄、有无取出内固定、复位质量、Garden分型等,临床治疗应依据患者自身情况,选取合适治疗方式。 Objective To analysis the risk factors of osteonecrosis of the femoral head after fixation (ONFH) treatment by cannulated compression screws for adult femoral neck fractures. Methods Retrospectively analysis the clinical and follow-up date of 120 patients with femoral neck fractures. All patients received fixation treatment by cannulated compression screws and the inci- dence of ONFH were calculated and related risk factors were analyzed by multivariate analysis. Results The outcome of Harris score assessment was increased after treatment than that before treatment( P 〈 0.0 5 ) . Multivariate logistic regression analysis showed the older age ( OR = 2.4 8, P 〈 0.05), removing internal fixator(OR = 10.11, P 〈 0.01 ), reduction quality (OR = 4.23, P 〈0.05 ) and higher Garden classification (OR = 1.64, P 〈 0.01 ) are potential risk factors of ONFH after fixation by cannulated compression screws. Conclusion The age, removing internal fixator, reduction quality and Garden classification are highly connected with ONFH after internal fixation, which should be considered carefully before treatment.
