摘要
目的探讨腹部倒立位X线片、瘘道造影及磁共振成像(MRI)影像学方法在儿童先天性肛门直肠畸形检查中的应用价值。方法回顾性分析我院2008年5月至2014年12月经手术确诊的先天性肛门直肠畸形患儿56例。患儿术前行腹部倒立位X线片56例,瘘道造影29例,MRI检查56例。结果 56例腹部倒立位X线片可以准确地显示肛门闭锁的位置,高位肛门闭锁的15例,中位闭锁的10例,低位闭锁的31例。29例瘘道造影可以准确地显示瘘管的走行和位置,其中会阴瘘18例,伴膀胱瘘6例,其他类型瘘5例;56例MRI可以清晰地显示肛周肌群发育情况及其他系统畸形,其中42例肛周肌群存在发育异常,脊柱、脊髓病变6例,右肾缺如1例。结论腹部倒立位X线片可以判断肛门直肠畸类型,瘘道造影对肛门直肠畸伴发的瘘管显示价值较大;MRI可以很好地显示肛周肌群发育情况及其他系统畸形,多种影像学方法结合在儿童先天性肛门直肠畸形检查中具有很高的临床应用价值。
Objective To investigate the clinical value of inverted plain film, fistula angiography and MRI imaging methods in congenital anorectal malformation （CARM）. Methods Fifty-six cases with operatively proved anorectal malformation from May 2008 to December 2014 in our hospital were reviewed. Abdominal plain film inverted 56 cases, 29 cases of fistula angiography, MRI examination 56 cases before surgery. Resuts Fifty-six cases of inverted abdominal plain film can accurately display the location of anal atresia, 15 cases of high anal atresia, 10 cases of median atresia, 31 cases of low atresia. 29 cases of fistula angiography can accurately display the location of the fistula and walk the line, in which 18 cases of perineal fistula, with six cases of bladder fistula, other types of fistula 5 cases; 56 cases MRI can clearly show the development of the situation anal muscles and other malformations presence dysplasia, spine, spinal cord lesions in 6 cases, kidney agenesis one case where 42 cases of anal muscles, the perianal muscles of which 36 cases dysplasia. Conclusion Abdominal inverted plain film can determine the type of CARM. Fistula angiography for anorectal fistula associated with abnormal display of great value; MRI can show a good development situation perianal muscles and other system abnormalities, various imaging methods combined with high clinical value in children with congenital anorectal malformations examination.
