摘要
目的通过对慢性非萎缩性胃炎(Chronic Non-Atrophic Gastritis,CNAG)内镜下黏膜改变与中医辨证分型相关性进行调查研究、总结规律,使内镜下辨病与中医辨证相结合,拓展中医四诊合参内涵,更好地明确疾病分型,提高临床诊断水平,并指导临床治疗用药。方法临床收集符合纳入标准的CNAG患者275例,记录患者内镜下胃黏膜表现与临床资料,并进行中医辨证分型,进行数据录入,采用SPSS17.0统计软件处理分析。结果慢性非萎缩性胃炎内镜下胃黏膜表现在各中医证型之间有显著差异(P<0.05)。本次研究的275例患者主要以脾胃虚弱证患者最多,其次为肝郁气滞、脾胃湿热、肝胃郁热、胃阴不足;胃络瘀阻证患者最少。脾胃虚弱证以胃镜下粘膜水肿为主要表现,肝郁气滞证主要表现为黏膜红斑,脾胃湿热证及肝胃郁热证则以充血及出血点或糜烂为主,胃阴不足证以黏膜粗糙为主。结论慢性非萎缩性胃炎内镜下胃黏膜表现与各中医证型之间存在一定的相关性和规律性。
Objective Through the correlation between mucosal changes under endoscopy and TCM syndrome differentiation of Chronic Non-Atrophic Gastritis（ CNAG）, we summed up the law, made the endoscopic disease differentiation and syndrome differentiation of traditional Chinese medicine combined with each other, expanded the connotation of four diagnostic methods of TCM, clear the disease classification better, improved the level of clinical diagnosis and guided clinical medicine. Methods Collecting 275 cases with CNAG patients who met the inclusive criteria, we recorded the clinical data of patients with endoscopic gastric mucosal manifestations, classified TCM syndrome, entered the data and used SPSS17.0 statistical software to process analysis. Results CNAG mucosal changes under endoscopy endoscopic showed no significant difference between the TCM type（ P〈0.05）. During 275 patients in the study, patients with spleen and stomach weakness were in the majority, followed by stagnation of liver qi, spleen stomach damp heat, stagnated heat of liver and stomach, stomach yin deficiency; blood stasis syndrome in patients with a minimum of stomach meridian. Spleen stomach deficiency in gastroscopic mucosal edema, liver qi stagnation syndrome mainly mucosal erythema, spleen stomach damp heat syndrome and the stagnation of liver and stomach heat syndrome with congestion and hemorrhage or erosion, deficiency of stomach yin deficiency by mucosal rough based. Conclusion There was correlation and regularity between the gastric mucosal expression and TCM syndrome types of CNAG under endoscope.
