摘要
本研究先生13名成年人正常(牙合)作颞下颌关断层X线摄片,每人摄片4张,共52张,将每张摄下的关节片放大3张,用数字化仪计算机测算放大的颞下颌关节X线片,包括面积测量(关节前间隙面积,后间隙面积)和线性测量(髁突在关节凹内的相对高度),经重复测量方差分析与可靠性方差分析后,证实本实验采用的髁突位置的测量方法具有重复性与可靠性,可用于髁突位置的测量研究。
The object of this investigation was to develop a method for using lateral TMJ tomogra-phy and joint space area measurements as well as linear measurements to make determina-tion of condyle positions.Such determination,it was believed,should have a quantitativeform.Corrccted lateral tomograms of the TMJs were made on a tomographic Philips polytomeU-3 machine,During this procedure the subject's head was stabilized in a specially designedhead-positioner.Both left and right TMJs were studied on each subject,providing a total of52 cases.Each tomographic image was enlarged in 6.25 ×.The condyle-fossa relationship was assessed in the swallowing position using the techni-que shown in Fig.2,6.A digital computer program was used to determine areas(to 0.01mm^2)of the anteriorand posterior joint spaces and the highest point(h)as well as(H)(to 0.1mm).The ratiosof P/A and hh′/HH′ were then calculated for each joint studicd.Error in the TMJ tracing procedure was believed to be the most important possibility formethodologieal inaccuracy.This type of error would be produced by failure of the observer toconsistootly interpret the tomographic images or by failure to draw the bony landmarks andeontours correctly.Tracing error was evaluated by means of several statistical measures(the P/A data and hh′/HH′data).Each of the 52 TMJs was traced 3 times,providing a total of156 hracing trials.A statistical method of“singlefactor experiments having repeated measur-es on the same elements”was performed from 156 tracings.The results have provided the following conlusions:1.Posteroanterior condyle position can be expressed in quantity,so can be done for upand down of condyle position.2.The reproducibility of the tracing procedure,the most critical element of the method,was high,in that condyle position differences between three tracings per joint were notsignificant at the 0.05 level of significance.3.The method presented has considerable usefulness because it well permit differentia-tions to be made between condyle pcsition,even when the changes are so slight as to beunobservable to the human eye.
