摘要
目的通过加强手术用血的审核管理,避免过度输血,节约有限的血液资源。方法制定手术用血管理规定,建立术中用血指征评估程序、取血审核管理流程,统计分析2013年6月前、后各1年的相关数据,比较评析加强审核管理前后的效果,不断探索手术用血新策略。结果对手术用血审核管理执行效果进行统计分析,结果显示,2013年6月-2014年5月与2012年6月-2013年5月相比较,手术备血台数基本持平(分别为483、486);备血量降低13.8%;术中用血手术的比例从59.3%降到38.9%;术中总用血量降低23.7%;台均用血量降低23.3%;计划用血与实际用血的符合性比较有明显降低;术中红细胞和血浆输注的合理性均明显提高(P<0.05)。结论加强对术中用血的审核管理,可有效减少各种血液成分的使用量,达到合理用血、节约用血的目的。
Objective To strengthen the audit management of blood transfusion in surgery in order to avoid excessive transfusions, and to reduce the use of the limited blood resources. Methods Management regulation was implemented for blood transfusion during surgery. An assessment system was established to evaluate surgical blood use and the procedures of procuring blood during operations. To further strategize on blood transfusion in surgery, statistieal analysis was conducted on the relevant data from June 2012 to May 2014, and the effect of the evaluation was investigated before and after the implementation of audit management. Results The results showed that the surgical number of blood preparation from June 2013 to May 2014, was similar to the number from June 2012 to May 2013, which were 483 vs 486. The quantity of blood for preparation dropped by 13.8%, while the actual use decreased from 59. 3% to 38.9%, while comparing the amount of quantity used in two years. Total numbers of blood transfusion in surgery decreased by 23.7%, while the average blood use during each operation also reduced by 23.3%. The preparation and the actual use of blood has seen a decrease in conformity. The rationality to use intraoperative red blood ceils and plasma transfusion has seen a significant improvement （P 〈 0.05）. Conclusion Through data analysis, strengthening the audit management of blood transfusion in surgery can effeetively reduce the use of all blood eomponents, and thus resulting in more reasonable and conservative blood transfusion.
