股骨近端锁定加压钢板治疗儿童股骨转子下骨折临床疗效分析

摘要 目的探讨股骨近端锁定加压钢板(LCP)治疗儿童股骨转子下骨折的临床疗效分析。方法回顾性分析2009年8月至2014年12月该院采用LCP治疗51例儿童股骨转子下骨折患儿的临床资料,记录所有患儿的手术时间、术中出血量、手术切口长度、术后引流量、住院时间及取钢板时间;综合评价骨折愈合及髋关节功能情况,髋关节功能按髋关节Sanders评分标准进行评分。结果所有患儿均获随访,随访时间5~47个月,平均28.5个月,所有患儿均未出现伤口感染、钢板及螺钉断裂、骨不连、肢体旋转畸形、髋内翻、股骨头坏死、再骨折等并发症。20例患儿拆除支具固定后出现膝关节屈曲功能受限,经过1~2周功能锻炼及下床适应后基本恢复正常。5例随访时间超过2年半的患儿出现患肢长于对侧(〈15 mm),但未出现明显跛行及长距离行走疲劳;1例患儿因手术切口出现瘢痕增生,但无明显瘢痕挛缩及日常活动受限。所选患儿中2例出现螺钉松动。髋关节Sanders评价结果显示:优46例,良3例,可2例,差0例。结论采用LCP治疗儿童股骨转子下骨折具有固定牢靠、治愈率高、并发症少,且符合儿童股骨近端解剖学特点,可作为治疗儿童股骨转子下骨折的选择之一。
