环氧乙烷消毒麻醉机对COPD吸入麻醉患者气道炎症反应的影响 被引量:3

摘要 目的:观察不同麻醉机消毒方法对手术合并COPD吸入性麻醉患者气道炎症反应的影响。方法:选取吸入全麻下行择期腹部手术且合并有慢性阻塞性肺病(COPD)的患者80例,随机分为A、B组,各40例,两组患者均行气管插管并行机械通气,术中行双腔导管左侧支气管内插管,A组患者使用术前经环氧乙烷浸泡消毒的麻醉机,B组患者使用术前采用2%戊二醛溶液浸泡消毒的麻醉机,测定两组插管后即时(T0)及插管后2h(T1)、4h(T2)血清、肺泡灌洗液(BALF)中的肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、白介素-2(IL-2)、白介素-6(IL-6)、白介素-8(IL-8)浓度。结果:两组血清细胞因子浓度随插管时间延长而升高,但各时间点血清细胞因子浓度比较无统计学差异(P>0.05);A组T0、T1、T2时IL-2浓度均显著低于B组(P<0.05);A组T1、T2时TNF-α浓度显著低于B组,T2时IL-8浓度显著低于B组(P<0.05)。结论:使用环氧乙烷对麻醉机进行消毒,对COPD吸入性麻醉患者气道炎症反应影响小,消毒效果优于2%戊二醛溶液,可起到一定的气道保护作用。
