有效评估糖尿病视网膜病变早期及非增殖期视网膜形态改变 被引量:5

摘要 目的:用光学相干断层扫描(optical coherence tomo-graphy,OCT)测定糖尿病病人的视网膜形态,并分别比较糖尿病视网膜病变早期(non-diabetes retinopathy,NDR)及非增殖期(non-proliferative diabetes retinopathy,NPDR)的视网膜形态上的变化,评价在不同分期下,糖尿病视网膜病变(Diabetic-retinopathy,DR)变化的严重程度。方法:选取50-71岁的老年正常人40人40只眼进行OCT测定黄斑区视网膜神经上皮层厚度作为OCT测定的视网膜正常形态的参考值;收集已确诊为2型糖尿病患者相关病例资料,并行眼底荧光血管造影确定分期后,在标准状态下行OCT检查,选取71人中的71只眼对结果进行统计学分析。结果:对于糖尿病这个危险因素,视网膜的功能在渐进性的下降,黄斑区神经上皮层在糖尿病视网膜病变早期(NDR)中变化不是很明显,而在糖尿病视网膜病变非增殖期(NPDR)的确变厚明显,有统计学意义。结论:OCT测定糖尿病视网膜病变黄斑区域的形态改变在疾病的早期观察、随访和进一步治疗的选择上具有很高的应用价值。
