摘要
目的分析中心医院指导下的社区管理模式对慢性心力衰竭患者的作用效果。方法慢性心力衰竭患者130例,按入院时间分为两组,A组为在中心医院指导下的社区慢性病管理模式,B组为常规社区医疗模式,比较两组心理健康水平、临床症状、体征及生活质量。结果治疗3个月后,A组心理健康水平明显高于B组(P<0.05或<0.01);6个月后,A组临床症状和体征、健康教育效果、治疗依从度和生活质量评分均高于B组(P<0.01)。结论中心医院指导下的社区管理模式对于慢性心力衰竭患者的社区治疗和康复有积极作用,不仅可以改善患者临床症状和体征,还可提高患者治疗依从性和健康教育效果,改善患者生活质量。
Objective To analyze the effect of community-based management for chronic heart failure patients under the guidance of the central hospital.Methods A total of 130 patients with chronic heart failure were divided into two groups,including A group with community-based management under the guidance of the central hospital and B group with regular community treatment.Mental health,clinical symptoms and signs and life quality were compared between two groups.Results After treatment of three months,the level of mental health was significantly improved in A group than in B group（P 〈0.05）.After treatment of six months,the scores of clinical symptoms and signs,effect of health education,treatment adherence and life quality were significantly higher in A group than in B group（P 〈0.01）.Conclusion The community-based management under the guidance of the central hospital has positive effects on the treatment and recovery of patients with chronic heart failure,which can not only improve the clinical symptoms and signs,but also raise treatment adherence,effect of health education and life quality.
