摘要
目的探讨200例孕妇阴道细菌感染对妊娠结局的影响。方法选择2014年6月至2014年11月于我院产科门诊就诊并诊断为细菌性阴道病(bacterial vaginosis,BV)的200例孕妇作为BV组,选择同期于产科门诊就诊的正常孕妇80例为对照组。取阴道分泌物做细菌涂片镜检及培养,并对140例BV孕妇进行乳酸菌阴道胶囊治疗,未治疗组患者60例。对所有孕妇的妊娠结局进行随访,并对不同妊娠结局的发生率进行统计学分析。结果 BV患者阴道分泌物一般为稀薄、匀质带有鱼腥味的灰白色分泌物,镜检可见线索细胞及少量白细胞。阴道菌群主要以加德纳菌为主。200例BV孕妇胎膜早破、绒毛膜羊膜炎、产后出血发生率均显著高于对照组,差异均有统计学意义(χ2=5.997,χ2=5.536,χ2=3.861,P均<0.05)。BV治疗组孕妇胎膜早破、绒毛膜羊膜炎和产后出血的发生率均明显低于未治疗组,差异均有统计学意义(χ2=4.668,χ2=10.992,χ2=4.344,P均<0.05)。结论孕期细菌感染可导致胎膜早破、绒毛膜羊膜炎、产后出血等,规范治疗可改善妊娠结局。
Objective To investigate the effection of bacterial vaginal infection to pregnancy outcome in 200 pregnancy. Methods 200 cases pregnancy with bacterial vaginosis （BV） as BV group, and 80 cases normal pregnancy as contorl group were collected in our hospital from June 2014 to November 2014. Vaginal secretions of all pregnancy were detected by smear and culture. 140 cases BV patients of BV group were treated by lactobacillus vagina capsule. All pregnancy were followed up , and all data were analyzed statistically. Re-sults Vaginal secretions of BV patients were grey, even consistent, thin, and fishlike smell. Microscopic ex-amination showed that clue cells and a few WBC. The main vaginal flora was Gardnerella vaginalis. The inci-dence rate of premature rupture of membrane, chorioamnionitis and postpartum hemorrhage in BV group were all higher than that of control group, the differences all had statistical significance （χ2=5.997, χ2=5.536, χ2=3.861, Pall〈 0.05）. The incidence rate of premature rupture of membrane, chorioamnionitis and postpartum hemorrhage in treatment group were all lower than that of untreated group , and the differences all had statisti-cal significance（χ2=4.668,χ2=10.992,χ2=4.344, Pall〈0.05）. Conclusion Bacterial infection during pregnan-cy can result in premature rupture of membrane, chorioamnionitis and postpartum hemorrhage. Standard treat-ment can improve the pregnancy outcome.
