摘要
目的探讨超低剂量数字X线摄影在手法复位小夹板固定治疗桡骨远端骨折中的应用价值。方法分别用常规剂量和超低剂量对200例桡骨远端骨折患者初诊和手法整复小夹板固定后复诊时进行X线检查,分别记录患者常规剂量和超低剂量X线摄影参数及剂量面积值(dose area product,DAP),进行比较,由两名高年资放射诊断高级职称医师对影像质量进行评价,并同时对结果进行统计分析。结果患者复诊X线检查时超低剂量曝光量值(0.90±0.14)显著低于初诊时常规剂量曝光量值(6.24±0.33)(t=13.65,P<0.01)。同样患者复诊时X线检查超低剂量的DAP(0.054±0.015)亦显著低于常规剂量的DAP(0.269±0.04)(t=10.13,P<0.01)。患者初诊及复诊时X线检查图像质量差异无统计学意义(Manne-Whitney U=1181.00,P=0.33)。结论超低剂量数字X线摄影可大大降低桡骨远端骨折手法整复小夹板固定后患者X线复诊时的曝光量和接受照射剂量,且对图像质量无影响。
Objective To study the value of low dose photography DR in manipulative reduction and small splinting fixation for the treatment of distal fractures of radius.Methods 200 patients with Manipulative reduction and small splinting fixation for the treatment of distal fractures of radius were performed with anterior-posterior wrist radiography with conventional dose for the first diagnosis,and with low dose for the follow-up.The difference of radiant exposure value and the dose area p roduct（DAP）between the two dose group s of radiography was analyzed.The imaging quality of the two dose group s was also evaluated in term of the subjective grading conducted by two radiologists.Results The radiant exposure value of the low dose group was 0.90±0.14,which was significantly lower than that of the conventional group（6.24±0.33）（t=13.65,P〈0.01）.The DAP of the low dose group was0.054±0.015,which was also significantly lower in comparison with that of the conventional group（0.269±0.04）（t=10.13,P〈0.01）.However,no significant difference in imaging quality was detected between the two dose groups（Manne-Whitney U =1181.00,P=0.33）.Conclusion The low dose wrist radiography could reduce markedly the exposure dose,whereas almost impact the imaging quality in manipulative reduction and small splinting fixation for the treatment of distal fractures of radius.
