摘要
目的探讨髂腹股沟入路术后淋巴漏的主要原因。方法新西兰大白兔10只,随机选择左右侧腹股沟区作为实验侧和对照侧,通过剥离髂血管周围淋巴管模拟临床牵拉伤,3d后应用亚甲蓝示踪了解表浅和深部淋巴显影情况。结果对照侧可见浅表及深部淋巴管和淋巴结显影良好,无亚甲蓝漏出;实验侧中8例显示髂外静脉周围淋巴漏,但髂总静脉处淋巴结和淋巴管未显影;2例显示髂外静脉周围显影剂无漏出,而髂总静脉处淋巴结显影。结论髂腹股沟入路术后淋巴漏的主要原因为术中移动髂血管时对深淋巴结和淋巴管造成了损伤,应对其一些步骤进行改良以避免术后淋巴漏的发生。
Objective To explore the main reasons of lymphatic leakage after ilioinguinal-approach surgery. Methods This study consisted of 10 New Zealand white rabbits, left or right side of Groin was selected in random to serve as experimental side and control side. The lymphatic vessels surrounding the lilac blood vessels were dissected to imitate the clinical distraction injury. The development of superficial and deep lymph vessels were observed 3 days later using methylene blue tracing. Results The images of superficial and deep lymph nodes were well developed in the control side, with no leakage of methylene blue was seen; in the experimental side, lymphatic leakage was observed around external iliac vein in eight cases, and lymph nodes and lymph vessels of common iliac vein were not stained by methylene blue, two cases showed no developing agent leaking out around the external iiiac vein, an image of lymph nodes of common lilac vein could be seen. Conclusion The main reason of lymphatic leakage after ilioinguinal approach surgery is damage of deep lymph nodes and lymph vessels in the process of mobilizing the iliac vessels, to avoid postoperative lymphatic leakage, some of its procedures should be improved.
