摘要
目的：探讨米力农对老年顽固性心力衰竭患者血流动力学的影响。方法选取2012年1月~2014年6月本院收治的60例老年顽固性心力衰竭患者作为研究对象，随机分为硝普钠组和米力农组，各30例。硝普钠组给予硝普钠0.5~3μg/（kg·min）静脉维持，米力农组给予米力农0.5μg/（kg·min）静脉维持，连续使用3 d，行脉搏指引持续心排出量监测（PICCO），观察血流动力学变化。结果米力农组的总有效率和有效率显著高于硝普钠组，差异有统计学意义（P〈0.05）。米力农组治疗后第1天的HR显著低于硝普钠组，差异有统计学意义（P〈0.05）。米力农组治疗后第1、2、3天的MAP显著低于硝普钠组，差异有统计学意义（P〈0.05）。米力农组治疗后第1、2天的CI、dPmax显著高于硝普钠组，差异有统计学意义（P〈0.05）。米力农组治疗后第1、2天的GEDI、SVRI显著低于硝普钠组，差异有统计学意义（P〈0.05）。结论米力农能够改善老年顽固性心力衰竭患者的心功能。
Objective To explore the influence of milrinone on hemodynamics in elderly patients with refractory heart failure. Methods 60 elderly patients with refractory heart failure from January 2012 to June 2014 in our hospital were selected and randomly divided into the sodium nitroprusside group and the milrinone group,30 cases in each group.The sodium nitroprusside group was given intravenous maintenance of sodium nitroprusside,the dosage was 0.5-3μg/（kg·min）. The milrinone group was given intravenous maintenance of milrinone,the dosage was 0.5 μg/（kg·min）,and the time of continuous use was 3 days.All patients were monitored by PICCO,and the hemodynamic changes was observed. Results The total effective rate and effective rate in milrinone group was higher than that in sodium nitroprusside group,with significant difference （P〈0.05）.HR in milrinone group at the first day after treatment was lower than that in sodium ni-troprusside group,with significant difference （P〈0.05）.MAP in milrinone group at the first,second and third day after treatment was lower than that in sodium nitroprusside group,with significant difference （P〈0.05）.CI and dPmax in milri-none group at the first,second day after treatment was higher than that in sodium nitroprusside group,with significant difference （P〈0.05）.GEDI and SVRI in milrinone group at the first,second day after treatment was lower than that in sodium nitroprusside group,with significant difference（P〈0.05）. Conclusion Milrinone can improve the heart function in elderly patients with refractory heart failure.
