摘要
目的：探讨社区护理理论结合家庭医生责任制服务模式及服务对象设计实施适合家庭医生责任制服务的社区护理模式。方法选取2012年6月至2013年6月签约居民中选择符合入选条件的家庭作为研究对象，随机分为常规组和观察组，每组各100户。常规组按现有护理模式给予护理；观察组制定合理的社区护理模式并给予针对性的护理，针对开展存在问题不断进行整改。两组实施半年后进行相关问卷调查，统计分析两组家庭中的老年及慢病成员自我效能情况、儿童计划免疫接种率和体检管理率及家庭满意度。结果观察组老年及慢病成员自我效能评分较常规组有明显提高（P＜0．05），0～6岁儿童计划免疫接种率及0～3岁体检管理率优于常规组（P＜0．05）；满意度优于常规组（P＜0．05）。观察组在诊疗、健康教育信息及活动、自我效能感知及儿童体检方面满意度明显优于常规组（ P＜0．05）。结论社区护士通过这种护理模式将疾病预防、保健、健康教育、康复等健康促进的功能和对社区、家庭甚至社区整体的护理功能融合完善，从而更好地服务于社区居民。
Objective To design and implement the community care mode, which suits for family doctor responsibility through community nursing theory with the family doctor service mode and service objects accountability.Methods The family complied with the provisions were selected as research subjects and divided into conventional and observation groups, 100 in each group.Family in convention-al group were given by existing models of care, in observation group were given community nursing care mode, and mode was rectified with the following problems.Six months later, associated questionnaires were collected, family members of the elderly and chronic disease self-ef-ficacy, the child immunization coverage rate, the rate of medical management and family satisfaction were compared.Results Self-efficacy score in observation group was significantly improved compared with that of conventional group, the difference was significant （ P〈0.05 ） . 0-6 years old children inoculated rate （ 100.00%） and 0-3 years medical management rate （ 97.03%） were better in observation group than those of conventional group.Satisfaction rate was 83.0%in conventional group, was 96.0%in observation group, the observation group was better than that of conventional group （P〈0.05） .Conclusions With this mode, nurse could connect health promotion functions such as disease prevention, health care, health education, rehabilitation and so on with communities, families and even whole communities care functional, and better serve the community.
