摘要
患者女,76岁。面部红斑,伴微痛1个月。皮肤科情况:双侧面颊融合成掌心大小暗红色斑块,稍有肿胀,压之稍有疼痛。其上散在分布少许针尖至小米粒大小紫癜,斑块上覆以少许细薄鳞屑,皮疹大致对称分布,皮损组织病理示:表皮棘层细胞间水肿,真皮内血管壁纤维素样变性,血管周围淋巴细胞、嗜酸性粒细胞浸润。直接免疫荧光:C3血管周围阳性,Ig G,Ig M和Ig A阴性。诊断:血管炎型药疹。
A 76-year-old female presented with erythema with a little pain on the cheeks for i month. Dermatological examination showed palm-sized dark red swollen plaques with slight touch pain, symmetrically distributed on the cheeks. Needle- to millet-sized purpuras were present on the plaques covered with fine scales. Histopathology showed intercellular edema in the stratum spinosum,and perivascular infiltration of lymphocytes and eosinophils. The fibrinoid degeneration was seen in the dermal vascular walls. Direct immunofluorescence showed C3 deposition around vessels. IgG, IgM, lgA were negative. A diagnosis of vasculitis induced by drug was made.
