摘要
目的 评价潜伏期行硬膜外阻滞分娩镇痛的效果及其对母婴的影响.方法 随机分配120例无产科及硬膜外麻醉禁忌症的单胎初产妇,在临产后,按照实施分娩镇痛时机不同分为A组：1 cm≤宫口＜2cm;B组：2cm≤宫口＜3cm,C组：3cm≤宫口＜5cm,每组40例.记录VAS评分、各产程时长、分娩方式、新生儿血气分析,体重及新生儿Apgar评分.结果 B组1例行剖宫产,各组间催产素使用量、第一、二产程时长、分娩方式、产后2h出血量、新生儿体重、脐静脉血气分析、Apgar评分比较差异无统计学意义（P＞0.05）.结论 潜伏期实施硬膜外分娩镇痛可提供产妇分娩过程中良好的镇痛效果,无延长第一、二产程时间,不增加产后2h出血,对新生儿Apgar评分无明显影响.
Objective To evaluate the effect of epidural labor analgesia during incubation period and its influence on maternal and neonatal.Methods 120 monomorphic primiparae without obstetric and epidural anesthesia contraindications were randomly divided into three groups according to the implementation time point of labor analgesia：group A：uterus cervix 1-2 cm,group B：uterus cervix 2-3 cm,group C：uterus cervix 3-5 cm,40 cases in each group.Recorded VAS score,delivery time,delivery mode,neonatal blood gas analysis,weight and neonatal Apgar score.Results There was only 1 case of cesarean section in group B.There were no statistically significant differences in oxytocin usage,first,second labor time,delivery mode,amount of bleeding in postpartum 2 h,birth weight,umbilical vein blood gas analysis,and Apgar score among three groups （P〉0.05）.Conclusions Epidural labor analgesia during incubation period can provide favorable analgesic effect in the process of childbirth,does not extend the first and second stage of labor,does not increase postpartum 2 h hemorrhage,has no obvious effect on neonatal Apgar score.
