摘要
目的探讨标准化沟通交接表在急诊科与ICU患者转运交接中的应用效果。方法制定标准化沟通交接表,2014年1—10月,在急诊科与ICU患者转入、转出时运用标准化沟通交接表交接患者,规范患者转运交接流程。结果运用标准化沟通交接表后,交接班科室医务人员对交班内容全面性、病情重点、管道管理、书写规范、交班语言表达、交接时间长短的满意率提高,分别达到94%、97%、95%、97%、99%、99%。结论运用标准化沟通交接表交接患者,规范了交接流程,缩短了交接时间。
Objective To explore the effect of the SBAR (Situation, Background, Assessment, Recommendation) communication mode based on de-escalation thinking on patient transfer between emergency department and ICU units. Methods Transition table covering the key process of SBAR was developed guided by de-escalation thinking and it was applied from January to October 2014 during patients transfer between ICU and emergency department. Results With the de-escalation thinking-based SBAR communication mode, medical staff from two departments were more satisfied with the patients ’ transfer with regard to detailed transfer process, the focus of the disease, standardized record, catheter management, fluent communication in transfer and the length of transfer time. Conclusion SBAR communication mode based on de-escalation thinking is effective for the standardization of patient transfer and the improvement of nursing efficiency.
