不同类别氟喹诺酮类药物对老年耐多药肺结核的疗效评价 被引量:19

Evaluation of curative effects of different fluoroquinolones on multi-drug resistant pulmonary tuberculosis in senile patients

摘要 目的探讨不同类别氟喹诺酮类药物对老年耐多药肺结核的疗效。方法收集我院结核科门诊复治的肺结核患者80例,随机分为左氧氟沙星组与莫西沙星组,各40例。两组基础治疗方案相同(选取帕司烟肼、利福喷丁、丙硫异烟胺、对氨基水杨酸钠、阿米卡星、吡嗪酰胺中4类或4类以上治疗),莫西沙星组加用莫西沙星400 mg,1次/d;左氧氟沙星组加用左氧氟沙星600 mg,1次/d,总疗程为12个月。记录两组抗结核治疗6个月和12个月时的疗效及不良反应。结果莫西沙星组与左氧氟沙星组治疗6个月时的总有效率分别为35.0%、12.5%,12个月时的总有效率分别为62.5%、32.5%,两组间差异均有统计学意义(P<0.05);治疗6个月时不良反应发生率分别为5.0%、12.5%,12个月不良反应分别为12.5%、27.5%,两组间差异无统计学意义(P>0.05)。结论莫西沙星联合其他抗结核药物治疗老年耐多药肺结核疗效可靠,明显优于左氧氟沙星。 Objective To explore the curative effects of different fluoroquinolones on multi-drug resistant pulmonary tuberculosis in senile patients, blethods 80 patients with pulmonary tuberculosis to receive retreatment in the Outpatient Department of our hospital were randomly divided into levofloxacin group and moxifloxacin group (n = 40 per group). The same basic treatment plan was applied to the patients in both groups (i.e., four or more medicines were selected from pasiniazide, rifapentine, ethionamide, sodium aminosalicylate, Amikacin, and pyrazinamide for the treatment). Besides, 400 mg of moxifloxacin was applied to the patients in moxifloxacin group (once every day) and 600 mg of levofloxacin was applied to the patients in levofloxacin group (once every day). The total course of treatment lasted for 12 months. The curative effects and adverse reactions of the patients were recorded after six months and 12 months of treatment, respectively. Results The overall efficacy in the two groups was 35.0 % and 12.5% after six months of treatment as well as 62.5% and 32.5% after 12 months, between which there was a statistical significance (P 〈 0.05); the adverse reactions was 5.0 % and 12.5% after six months of treatment as well as 12.5% and 27.5% after 12 months, between which there was not a statistical significance (P 〉 0.05). Conclusion Moxifloxacin in combination with other anti-tuberculosis drug has reliable curative effects on muhi-drug resistant pulmonary tuberculosis in senile patients, and the curative effect is obviously better than that of levofloxacin.
