摘要
目的：探讨选择性痔上黏膜切除钉合术（ TST）治疗Ⅱ~Ⅲ度痔的临床疗效。方法将我院自2012年9月以来收治的120例Ⅱ~Ⅲ度痔患者随机分为试验组（ TST组）和对照组（痔上黏膜环切钉合术，PPH组）各60例，对比其手术疗效和术后并发症。结果两组平均手术时间、平均住院时间及总体满意度TST组优于PPH组。两组术后疼痛、吻合口狭窄比较差异无统计学意义（P﹥0.05）；术后出血、尿潴留、肛缘水肿、肛门坠胀比较差异有统计学意义（P﹤0.05）。结论TST作为痔治疗的一种新手术，手术疗效确切，手术创伤小，术后恢复快，符合现代外科微创发展要求，值得临床推广应用。
Objective To investigate the clinical effects of tissue-selecting therapy stapler（ TST） in the treatment of gradeⅡandⅢhemorrhoids. Methods A total of 120 cases of gradeⅡandⅢhem-orrhoids in our hospital were randomly divided into experimental group（ TST,n =60 ）and control group （PPH,n=60）from September 2012. The surgical efficacy and postoperative complications between two groups were compared. Results The TST group was superior to the PPH group in mean operation time, mean hospital stay and total satisfaction. While TST group had less postoperative bleeding,urinary reten-tion,anal margin edema,and anal swelling sense（P﹤0. 05）. However,there were no differences in post-operative anal pain or anastomotic stricture between the two groups（P﹥0. 05）. Conclusion As a novel therapy,TST has good operative effects,little trauma,and rapid recovery after operation,which is suitable for the development of modern minimally invasive surgery. It is worthy of being spread in clinical applica-tion.
