摘要
上海是我国进行首批医改试点的城市之一,由于地域经济发达、治疗水平领先、文化底蕴深厚与较强的自我保健意识决定了上海地区的用药特点。本文使用近5年上海地区样本医院药品购入总金额增长幅度与全国22城市地区样本医院的数据对比,从用药大类的份额占比变化、医院使用领先药品及主要供应商位序、药品的来源构成与生产商的主打品种等维度研究医院用药品种的变化及厂商的竞争情况。政策与制度的不断出台左右了上海地区医院用药,上海地区医院无论在用药水平、临床品种选取或药品来源的选择等方面都有其特殊性,招标规范与用药的合理性使合资药与国产药的竞争难分伯仲,目前销售领先的厂商及其主打品种的地位难以撼动。
Shanghai is one of the first batch of pilot cities to implement Chinese medical reform. The developed regional economy, leading therapeutic levels, rich cultural heritage and strong sense of self-care determine the medication characteristics of Shanghai. The change of drugs used in hospital and the competition status among manufacturers were studied by comparison of the data on the growth rate of total amount of drugs purchased in the sample hospitals of Shanghai in the past 5 years with those in 22 sample hospitals in other urban areas of China in terms of the change of the share of drug category, leading drugs from hospitals and a major supplier of rank and the sources of drugs and the manufacturer＇s flagship species. The drugs used in the sample hospitals of Shanghai were affected by the continuous introduction of the policies and institutions. The hospitals in Shanghai have their particularity in terms of the level of medication, the selection of the clinical varieties and the source of the drugs. There is no difference between the joint-stock and home-made drugs in doctor＇s prescription behavior because of the standardization of bidding and the rational use of drugs. At present the dominant positions in the market are difficult to beat for the lending pharmaceutical companies and their major products.
