摘要
目的:研究吉西他滨联合奥沙利铂介入栓塞治疗原发性肝癌合并肾功能衰竭代偿期的疗效。方法:将2013年5月~2014年9月期间我院收治的90例原发性肝癌合并肾功能衰竭代偿期患者纳入研究,随机分为两组,观察组患者接受吉西他滨联合奥沙利铂介入栓塞治疗、对照组患者接受吉西他滨联合奥沙利铂静脉化疗,比较两组患者的血清肿瘤标志物、黏附分子含量以及肾功能指标。结果:(1)肿瘤标志物:观察组患者血清高尔基体蛋白73(GP73)、血管内皮生长因子(VEGF)、缺氧诱导因子-1α(HIF-1α)含量低于对照组;(2)黏附分子:观察组患者血可溶性细胞间黏附分子-1(sICAM-1)和可溶性血管黏附分子-1(sVCAM-1)、高迁移率族蛋白1(HMGB1)含量低于对照;(3)肾功能指标:观察组患者的尿β2微球蛋白(β2-MG)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)以及血清中胱抑素C(Cys C)含量均低于对照组。结论:吉西他滨联合奥沙利铂介入栓塞治疗能够更为有效的杀灭肝癌细胞、降低血清肿瘤标志物和黏附分子含量,同时减小对肾功能造成的损害,是治疗原发性肝癌合并肾功能衰竭代偿期的理想方法。
Objective: To study the curative effect of gemcitabine combined with oxaliplatin interventional embolization in treating primary liver cancer eomplicated with compensatory stage renal failure. Methods: 90 cases of primary liver cancer complicated with compensatory stage renal failure in our hospital from 2013 May to Z014 September were enrolled and random ly divided into two groups. Observation group received gemcitabine combined with oxaliplatin interventional embolization treat- ment, control group received gemeitabine combined with oxaliplatin intravenous chemotherapy therapy. Then serum tumor marker compound, adhesion molecule content and renal function indices were compared. Results= Serum GP73, VEGF, HIF- 1α contents of observation group were lower than those of control group; Serum sICAM-1, sVCAM-1, HMGB1 contents of observation group were lower than those of control group; Urine J32-MG, NGAL and serum CysG content of observation groupwere lower than those of control group. Conclusion: Gemeitabine combined with oxaliplatin interventional embolization treat- ment can more effectively kill hepatocellular carcinoma cells, reduce serum tumor markers and adhesion molecules contents, decrease renal function damage; it's an ideal method in treating primary liver cancer complicated with compensatory stage renal failure.
