摘要
目的探讨抗核小体抗体等自身抗体在系统性红斑狼疮诊断中的应用价值。方法收集196例SLE患者,129例其他风湿病患者和51例健康对照者血清,并用免疫印迹法对3组血清进行抗核小体抗体(Anu A)、抗ds-DNA抗体、抗Sm抗体等自身抗体进行检测。同时对SLE组的临床指标和其他检测结果进行分析,并对SLE组和疾病对照组的Anu A和其他自身抗体进行比较分析。结果 SLE组Anu A的阳性率明显高于其他风湿病组,健康对照组Anu A均为阴性;Anu A在SLE组中的敏感度和特异度分别为63.3%和91.3%,Anu A的敏感度较抗ds-DNA抗体和抗Sm抗体高,差异有统计学意义(P<0.05)。Anu A的特异度高于ANA和组蛋白,差异有统计学意义(P<0.05),Anu A、抗dsDNA抗体及抗Sm抗体联合检测的敏感度和特异度分别为89.8%和94.6%,其敏感度和特异度均较高。SLE疾病活动期的Anu A及其他自身抗体的阳性率明显高于SLE疾病非活动期,且差异有统计学意义(P<0.05)。结论 Anu A等自身抗体可能参与了SLE的发病过程,联合检测Anu A等多种自身抗体,对SLE的正确诊断、病情判断和疗效评估有重要意义。
Objective To explore the significance and value of the anti-nucleosome antibodies in the diagnosis of Systemic Lupus Erythematosus( SLE). Methods To detect the nucleosome antibodies( Anu A) in the collected serum of 196 patients with SLE,129 patients with other rheumatic diseases and 51 healthy controls by Western blot. To analyze the clinical manifestations and other test results of SLE patients simultaneously,and analyze Anu A and other autoantibodies between SLE group and the control group. Results The positive rate of Anu A in SLE group was significantly higher than that in other rheumatic diseases and Anu A was all negative in control group; Sensitivity of Anu A in SLE group was 63. 3% and specificity was 91. 3%; Sensitivity of Anu A was significantly higher than that of the anti-ds-DNA antibodies and anti-Sm antibodies. The difference was statistically significant with comparison( P 0. 05). Sensitivity and specificity of joint detection including Anu A,anti-ds-DNA and anti-Sm antibodies were respectively 89. 8% and 94. 6%,which were significantly higher. Positive rates of Anu A in active period of SLE were significantly higher than that in the non-active period,and higher than the ds-DNA antibodies( P 0. 05). Conclusion Joint detection of autoantibodies like Anu A might be of importance in the correct diagnosis,the condition judgment and efficacy evaluation of SLE.
