摘要 目的探讨全胸腔镜下肺叶切除术治疗老年非小细胞肺癌的的临床应用,全胸腔镜微创治疗的适应症及手术方式及术后恢复。方法采取胸腔镜手术组84例,左肺上叶14例(其中袖式支气管肺叶切除2例),左肺下叶切除16例,右肺上叶切除24例(其中袖式支气管肺叶切除4例),右肺上中叶切除2例,右肺下叶切除24例,右肺中叶切除2例,右肺中下叶切除2例。结果 80例在电视胸腔镜下完成手术,患者无围术期死亡。未出现术后支气管胸膜瘘、脓胸、肺不张等并发症。结论全胸腔镜(VATS)肺叶切除技术能让更多老年肺癌患者接受外科治疗,达到治疗的微创化。 Objective To discuss the clinical value of video-assisted thoracoscopic pulmonectomy in elderly patients with non-small-cell lung cancer and the major Indications. Method VideoAssisted Thoracoscopic Surgery for 84 cases. lobectomy. Included 14 cases on the left upper lobe(1 case with Sleeve resection of the left upper lobe) and 16 cases on the left lower lobe.24 cases on the right upper lobe(4 cases with Sleeve resection of the right upper lobe),2 case on the right upper and middle lobe, 24 cases on the right lower lobe, 2cases on the right middle lobe, 2 cases on the right middle and lower lobe.Results 80 cases were finished by Video-Assisted Thoracoscopic Surgery.4 case were finished by thoracoscopic-Assisted mini-incision pulmonary lobectomy.They all recovered well.No patient showed bronchopleural fistula, tempyema and atelectasis. There was no perioperative death. Conclusions Video-assisted thoracoscopic surgery(VATS) has some advantage including safty,fast recovering,and minimally invasion.It's applied to elderly patients with lung cancer that need lobectomy.
