PD-1在HIV合并肺结核患者免疫功能中的作用 被引量:4

Significance of PD-1 in HIV and pulmonary tuberculosis co-infected patients

摘要 目的探讨HIV合并肺结核患者T淋巴细胞上程序性死亡分子(PD-1)及其配体(PD-L1)表达的特性。方法选取2012年2月至2014年5月在江门市医院就诊的单纯HIV感染者30例(A组)、单纯肺结核患者29例(B组)、HIV合并肺结核患者29例(C组)、健康对照组30例(D组),检测各组外周血CD4+T淋巴细胞计数、血浆HIV病毒载量、CD4+T和CD8+T淋巴细胞上PD-1及PD-L1表达水平,各组进行比较。结果在CD4+T淋巴细胞计数方面,C组[(175.34±52.16)/μl]<A组[(353.25±68.64)/μl]<B组[(578.66±102.27)/μl]<D组[(829.34±257.58)/μl],每2组之间差异均有统计学意义(P<0.05)。血浆HIV病毒载量方面,C组(320 000 copy/ml)明显低于A组(13 540copy/ml),差异有统计学意义(P<0.05)。在CD4+T和CD8+T淋巴细胞PD-1水平及PD-L1水平方面,A/B/C组均较D组明显升高,差异有统计学意义(P<0.05)。C组较A组、B组更高,差异有统计学意义(P<0.05)。A组、B组两组间比较差异无统计学意义。结论 CD4+及CD8+T淋巴细胞PD-1及PD-L1水平可能同HIV和肺结核合并感染者的免疫功能低下有关。 Objective To explore the expressions of programmed death-1 (PD-1) and its ligand PD-L1 in HIV and pulmonary tuberculosis co-infected patients. Methods HIV infected patients (group A, 30 cases),patients with pulmonary tuberculosis (group B,29 cases), HIV and pulmonary tuberculosis co-infected patients (group C,29 cases), and controls (group D,30 cases) were randomly included in this study. Peripheral blood CD4+ T cells, HIV-RNA levels and the expressions of PD-1/PD-L1 on the peripheral blood CD4+/CD8+ T lymphocytes were detected and compared. Results There were significantly difference in CD4+ T cells between any two groups [ group C ((175.34±52.16)/μl) 〈 group A ((353.25± 68.64)/μl)〈group B ((578.66±102.27)μl)〈group D ((829.34±257.58)/μl), (P〈0.05)]. Serum HIV-RNA level in group C(320 000 copy/ml) was significantly lower than that in group A(13 540 copy/ml) (P〈0.05). PD-1 and PD-L1 in both CD4+ T and CD8+ T lymphocytes in group A/B/C were significantly higher than those in group D (P〈0.05), and those in group C were significantly higher than those in group A/B (P〈0.05), while there were no statistically differences between group A and group B. Conclusion PD-1/PD-L1 on the CD4+/CD8+ T lymphocytes may relate to the immunodeficieney of HIV and pulmonary tuberculosis co-infected patients.
