结直肠癌根治术后血清癌胚抗原变化趋势与患者预后的关系被引量：4

摘要目的探讨结直肠癌根治术后癌胚抗原(CEA)的动态变化趋势与结直肠癌复发、预后的关系。方法165例结直肠癌根治术后患者,术后根据CEA变化趋势分为快速上升型45例(快速组)和缓慢上升型120例(缓慢组),分析CEA变化趋势与直肠癌临床病理特征及患者预后的关系。结果快速组淋巴结转移率明显高于、TNM分期为Ⅲ期者所占比例、术前CEA>5μg/L者所占比例明显高于缓慢组,P均<0.05。快速组复发转移率(86.7%)明显高于缓慢组(25%);多病灶转移率(76.9%)明显高于缓慢组(60.0%),P均<0.05。快速组Ⅲ期者复发转移率和病灶转移率更高,可再次行R0切除术的几率极小。快速组五年生存率低于、中位生存时间短于、中位无进展生存期短于缓慢组,P均<0.01。结论 CEA的动态变化可提示结直肠癌患者预后。
