摘要
目的探讨MRI扩散加权成像(DWI)、表观扩散系数(ADC)值用于诊断第四脑室原发脑膜瘤的可行性,以提高影像诊断准确性,为治疗方案的制定提供影像依据。方法回顾分析7例经手术和病理学证实的第四脑室原发脑膜瘤患者的DWI、ADC图像表现,测量计算ADC值,与文献报告的脑膜瘤ADC值进行比对,同时结合常规MRI表现进行影像诊断。结果该组脑膜瘤的平均ADC值有6例在(1.159±0.037)×10-3 mm2/s与(1.173±0.062)×10-3mm2/s之间,1例平均ADC值为(0.765±0.036)×10-3 mm2/s,经配对样本t检验,P=0.013(<0.05),与文献报告的第四脑室良性脑膜瘤ADC值(1.171±0.214)×10-3 mm2/s,恶性脑膜瘤平均ADC值(0.752±0.215)×10-3 mm2/s相一致,6例符合典型脑膜瘤,1例符合非典型/恶性脑膜瘤。结论利用DWI和ADC值提供的诊断信息和常规MRI成像相结合,可以提高第四脑室原发性脑膜瘤影像诊断准确率。
Objective To investigate the diffusion-weighted imaging (DWI), apparent diffusion coefficient (ADC) values for the diagnosis of primary meningioma feasibility of the fourth ventricle, in order to improve the accuracy of diagnostic imaging, providing video evidence for treatment programs. Methods A retrospective analysis of seven cases confirmed by surgery and pathology of the fourth ventricle in patients with primary meningioma DWl, ADC maps performance measarement and calculation ADC values, ADC values previously reported meningioma were compared, combined with the performance of conventional MRI diagnostic Imaging. Results The mean ADC value of the group of six cases of meningioma was between (1.159±0.037)×10^-3 mm^2/s-and (1.173±0.062)×10^-3mm^2/s, one patient with a mean ADC value (0.765±0.036)×10^-3 mm^2/s, by paired sample t test, P=0. 013 (d0. 05), statistically significant, this result was consistent with the previous report of the fourth ventricle benign meningioma ADC value (1.171±0.214)×10^-3 mm^2/s and malignant meningeal average ADC values (0.752±0.215)×10^-3 mm^2/s, six cases were accord with the typical meningioma, one case was in line with atypical / malignant meningioma. Conclusion DWI and ADC values provide diagnostic information and can be combined with conventional MRI imaging to improve the image of the fourth ventricle meningioma primary diagnostic accuracy.
