多频生物电阻抗法在腹膜透析患者透析充分性评估中的价值 被引量:3

The value of multifrequency bioelectrical impedance analysis in assessing adequacy of peritoneal dialysis patients

摘要 目的探讨应用多频生物电阻抗法(MBIA)检测腹膜透析患者的人体成分及其分布情况,各成分与尿素清除率(Kt/V)的关系。方法 60例腹膜透析患者,按体质指数(BMI)分为低体重组(17例)、体重正常组(25例)、超重肥胖组(18例)。采用Inbody S10人体成分分析仪测量体重(WT)、BMI、总体水(TBW)、细胞外液(ECW)、细胞内液(ICW)、脂肪组织(FM)、去脂体重(FFM),同时计算Kt/V。结果超重肥胖组人体成分各指标均明显高于体重正常组(P<0.05或P<0.01),低体重组人体成分各指标均明显低于体重正常组(P<0.05或P<0.01)。超重肥胖组Kt/V明显低于正常组(P<0.01)。Kt/V与TBW、ECW、ECW/TBW、ECW/ICW均呈负相关(P<0.01)。结论不同BMI的腹膜透析患者的人体成分变化明显,MBIA可对透析充分性作出间接判断,有一定的临床应用价值。 Objective To measure the body compositions and their changes of peritoneal dialysis (PD) patients by multifrequency bioelectrical impedance analysis (MBIA) and compare the relative data with the peritoneal dialysis adequacy (Kt/V). Methods A total of 60 PD patients were divided into three groups according to body mass index (BMI) : underweight group (BMI〈18.5), normal weight group(BMI18.5-23.9), and overweight and obese group (BMI≥23.9). The patients' weight (WT), body mass index (BMI), total body water (TBW), extracellular water (ECW), intracellular water (ICW), fat mass (FM), and fat-free mass (FFM) were measured by MBIA. Urea clearanee rate (Kt/V) was estimated at the same time. Linear correlation were used to analysis between Kt/V and body composition indicators. Results Body composition indicators were significantly higher in overweight and obese group than in normal weight group(P〈0.05 or P〈0.01), and were significantly lower in underweight group than in normal weight group(P〈0.05 or P〈0.01). The Kt/V was significantly lower in overweight group than in normal weight group (P〈0.01). Kt/V was negatively correlated with TBW,ECW,ECW / TBW.ECW / ICW (P(0. 01). Conclusion Body composition is obviously different in PD patients with different BMI. MBIA appears to be a practical method for assessing fluid status and dialysis adequacy in PD patients.
