摘要
目的：观察大肠癌并发肠梗阻一期手术治疗的效果。方法选取31例大肠癌并发肠梗阻行一期手术治疗，其中接受左半结肠Ⅰ期切除吻合术共18例，右半结肠Ⅰ期切除吻合术共8例，直肠癌Melis术共5例。观察治疗效果、并发症、术后生活质量。结果随访1～5年，左半结肠Ⅰ期切除吻合术、直肠 Melis 术3年生存率（94．44％、100．00％）明显高于右半结肠Ⅰ期吻合术；左半结肠Ⅰ期切除吻合术5年生存率（94．44％）明显高于直肠Melis术和直肠Melis术。治疗后患者的身体功能、心理功能、社会功能、总体生活质量评分分别为（78．37±7．12）、（59．54±5．32）、（58．83±2．45）、（85．12±8．43）；术后发生伤口感染3例，肠粘连1例，并发症发生率为12．9％。结论一期手术选择合适的手术方式治疗大肠癌并发肠梗阻能够达到良好的治疗效果，改善患者生活质量。
Objective To study the efficacy of Ⅰ stage operation for colorectal cancer complicated with intestinal ob-struction.Methods 31 cases of colorectal cancer complicated with intestinal obstruction were given Ⅰstage operation,18 cases were given left half colon resection and anastomosis ,8 cases were given right half colon resection and anastomosis ,and 5 cases were given Melis rectal cancer surgery .Curative effects ,complications and postoperative life quality were observed .Results Fol-low-up was 1-5 years,3-year survival rates of patients treated with left half colon resection and anastomosis and Melis operation were 94.44%and 100.00%,which were significantly higher than patients who received right colon resection ;5-year survival rate of patients treated with left half colon resection and anastomosis was 94.44%,which was significantly higher than patients who re-ceived right colon resection and rectal Melis;After treatment , body function , psychological function , social function , and overall quality of life were（78.37 ±7.12）、（59.54 ±5.32）、（58.83 ±2.45）、（85.12 ±8.43）;there had 3 cases of postoperative wound infection ,1 intestinal adhesion , and the rate of complications was 12.9%.Conclusion AppropriateⅠstage operation treatment for colorectal cancer complicated with intestinal obstruction can achieve good effects ,and improve patients＇quality of life.
