摘要
目的了解四川省境内异地就诊患者出院人员来院复印病历的现状、目的，分析存在的问题，为建立省内医院信息系统（HIS）与异地医疗保险机构结算平台提供参考依据。方法利用自制调查问卷对曾在四川省最大两家三甲医院的330名复印病历的人员进行横断面调查，并对结果进行分析。结果80．6％（266／330）病历的复印目的为医疗保险，87．9％（290／330）为省内异地统筹，57．6％（190／330）的患者异地就医原因为转诊、转院治疗，95．2％（314／330）对异地就医报销现状不满意，94．5％（312／330）对建立省内HIS与异地医保联网结算平台表示支持，垫付费用困难与否是影响对建立联网及时结算平台看法的主要因素。结论应尽快构建区域内乃至省内HIS与医疗保险机构结算平台，并从体制、管理、技术、服务等层面加强监管，为参保人员提供优质便捷的综合服务。
Objective To understand the current situation and motives of xeroxing medical records, an- alyze the existing problems, and provide the reference for the establishment of Sichuan provincial hospital infor- mation systems （HIS） and remote medical insurance institution settlement platform. Methods A crosssection- al survey was carried out in 330 people who xeroxed medical records with self-designed questionnaires in two most largest first class hospitals in Sichuan province. Results 80.6% （266/330） people xeroxed medi- cal records for medical insurance. 87.9% （290/330） for long-distance medical insurances. The reason of 57.6% （190/330） of patients with medical referral was for medical treatment. 95.2%（314/330） were not satisfied with long-distance medical reimbursement status quo. 94.5%（312/330） of the respondents supported to es- tablish timely settlement platform between HIS and medical insurance institution in the province. Dis- bursement of difficulty or not is the main factor affecting the views of establishment of timely transaction platform. Conclusions Long-distance medical insurance institution transaction platform between HIS sys- tem and medical insurance institution should be established within the region or in the province, and strengthen the supervision from the aspects of system, management, technology and service, to provide high quality and convenient services for the insured.
