摘要
目的:探讨彩色多普勒超声检查诊治小儿肠套叠及在彩超引导下行水压灌肠复位术的价值。方法:使用彩色多普勒超声腹部探头及高频探头,对可疑肠套叠患者进行多切面探查,发现异常肠源性团块回声诊断为肠套叠时用水压灌肠法进行复位,术中在彩色多普勒超声引导下动态观察团块的大小、所处位置、回声变化及有无并发症,并摄片和录像记录,追踪随访。结果:采用腹部及高频彩色多普勒超声对小儿急性肠套叠能做出较明确的诊断;病程在48 h内的小儿急性单纯性肠套叠,在彩色多普勒超声引导下实时观察使用水压灌肠复位效果良好。结论:彩色多普勒超声引导下进行肠套叠水压灌肠复位能动态观察其活动情况,并了解有无并发症,其操作简单,成功率高,可为临床诊断及治疗小儿肠套叠首选。
Objective： To study the ultrasonic diagnosis of pediatric intussusception ultrasonographic features and the water pressure enema reduction under guided by ultrasound. Methods： Using abdominal probe and the high frequency probe, in patients with suspected intussusception, faceted searches found abnormal enterogenous briquette echo diagnosis of intussusception, reset by hydraulic pressure enema method, intraoperative ultrasound guided under dynamic observation block, the location and the size of the change of the echo, and presence of complications, and shoot, video records, follow-up. Results： The abdominal and high frequency color to exceed of infantile acute intussusception can make a definite diagnosis; Course of the disease within 48 h of infantile acute idiopathic intussusception in ultrasound guided by real-time observation using hydrostatic enema reduction effect is good. Conclusion： Ultrasound guided hydrostatic enema reduction intussusception can dynamically observe its activity and presence of complications. Operation is simple, high success rate, clinical diagnosis and treatment of intussusception is preferred.
