摘要
目的探讨旋后内收型踝关节骨折的手术方法和治疗效果。方法自2004年1月至2012年10月,我院共对39例旋后内收型踝关节骨折的患者进行了手术治疗,其中男23例,女16例;年龄20～65岁,平均45.15岁;其中左踝18例,右踝21例;1例为开放性骨折。所有患者均行切开复位内固定术,术后石膏托固定4周,定期复查。结果 32例术后获得平均随访16（10～24）个月。伤口均I期愈合,无感染。骨折愈合时间为10～16周,平均11.2周。患者术后平均14（11～18）周时可完全负重行走,无明显疼痛不适。术后采用美国骨科足踝协会（American orthopaedic foot and ankle society,AOFAS）踝关节-后足评分标准,评分为平均84.1（72～100）分。结论对旋后内收型踝关节骨折进行解剖复位和正确的内固定,是恢复踝关节正常形态及良好行走功能的基础。
Objective To investigate the surgical methods and effects in the treatment of supination adduction ankle fractures. Methods Thirty-nine patients of supination adduction ankle fractures were treated from January 2004 to October 2012. There were 23 males and 16 females with an average age of 45.15 years（ range： 20-65 years）. Eighteen cases with the left ankle fractures and 21 cases with the right ankle fractures were included. One of them was of open fractures. All patients were treated by open reduction, internal fixation and plaster external fixation for 4 weeks postoperatively. Results 32 /39 cases were followed up for 16 months in average（ range： 10-24 months）. All of the cases healed well at the first stage, and the mean time of bone union was 11.2 weeks（ range： 10-16 weeks）. No infection was found in all cases. The mean time of complete weight loading walking was 14 weeks（ range： 11-18 weeks）, without obvious pain. According to the Ankle Hindfoot scale of American Orthopaedic Foot and Ankle Society（ AOFAS）, the mean score was 84.1（ range： 72-100）. Conclusions Anatomic reduction and stable internal fixation are the best solutions for dislocated ankle fractures, which are the foundation of the reconstruction of ankle form and function.
