以氧合指数为控制窗的有创-无创序贯性机械通气在治疗ARDS中的应用 被引量:13

Application of oxygenation index window as switching for sequential invasive-noninvasive mechanical ventilation in treating ARDS

摘要 目的探讨以氧合指数(OI)为控制窗的有创-无创序贯机械通气在治疗急性呼吸窘迫综合征(ARDS)中的应用效果。方法 91例ARDS行机械通气患者随机分为观察组(以OI为控制窗,46例)和对照组(45例),比较两组相关监测指标的变化。结果与治疗前相比,两组治疗后OI均升高,HR、RR和PaCO2均降低(P<0.05)。治疗前后的HR、RR、MAP、PaCO2和OI两组差异均无统计学意义(P>0.05)。与对照组比较,观察组有创通气时间、总机械通气时间和住ICU时间较短,呼吸机相关性肺炎发生率和病死率较低(P<0.05)。结论以OI为控制窗的有创-无创序贯机械通气能够明显减少有创通气时间,改善患者预后。 Objective To investigate the efficacy of oxygenation index(OI)window as switching for sequential invasive-noninvasive mechanical ventilation in treating acute respiratory distress syndrome(ARDS).Methods A total of 91 patients with ARDS was randomized into two groups of A(taking OI window as switching for mechanical ventilation,46cases)and B(treated with conventional management,45cases).The indicators monitored were compared between two groups.Results Compared with before,OI was increased and HR,RR and PaCO2 were decreased after treatment in both groups(P〈0.05).HR,RR,MAP,PaCO2 and OI before and after treatment were not significantly different between two groups(P〉0.05).Compared to group B,the times of invasive ventilation,total mechanical ventilation and ICU stay were shorter,and incidence rate of ventilationrelated pneumonia and mortality were lower(P〈0.05).Conclusion Taking OI window as switching for sequential invasive-noninvasive mechanical ventilation can significantly reduce the time for invasive ventilation and improve the prognosis of ARDS patients undergoing mechanical ventilation.
